厦门市正常成人鼻腭管解剖及其变异的锥形束CT三维测量分析

摘  要:目的 采用锥形束CT对厦门市正常成人的鼻腭管解剖及其变异进行三维测量分析。方法 选取2020年1月至2023年1月到厦门医学院附属口腔医院就诊的102例患者的锥形束CT影像资料作为研究对象,制定在矢状面、冠状面以及水平面上的测量指标及标准,并根据年龄、性别对患者进行分组,以分析年龄、性别与各测量指标间的关系。结果 鼻腭管正中矢状面形态为漏斗状35例(34.3%),圆柱状33例(32.4%),沙漏状26例(25.5%),香蕉状8例(7.8%)。鼻腭管冠状面形态变异中,单管44例,双平行管2例,Y型管56例,其中2个Stenson孔者46例,3个及以上者10例。从水平面观察,仅有1个Stenson孔和切牙孔的鼻腭管43例,1个Stenson孔但有3个切牙孔者1例;有2个Stenson者48例(1个切牙孔者46例,2个切牙孔者2例);3个及以上的Stenson孔10例(3个Stenson孔6例,4个3例,5个1例),均仅有1个切牙孔。按性别分组:鼻腭管上口直径D1、下口直径D3、长度L1、唇侧骨板低位厚度W2、鼻腭角(A)2组间差异具有统计学意义(P<0.05),余无统计学意义(P>0.05);按年龄分组:鼻腭管中位直径D2、下口直径D3、唇侧骨板高位厚度W1、低位厚度W2三组间差异具有统计学意义(P<0.05),其余差异无统计学意义(P>0.05)。结论 厦门市正常成人鼻腭管解剖变异多见,鼻腭管上、下口直径、长度、唇侧骨板低位厚度及鼻腭角与性别相关,鼻腭管中位、下口直径、唇侧骨板高位、低位厚度与年龄相关。Objective To analyze the anatomy and variation of the nasopalatine canal in normal adults in Xiamen by cone-beam CT.Methods The cone-beam CT image data of 102 patients who visited the Affiliated Stomatology Hospital of Xiamen College from January 2020 to January 2023 were selected as the research objects,and the measurement indexes and standards in sagittal plane,coronal plane,and horizontal plane were formulated,and the patients were grouped according to age and gender.The relationship between age,sex,and each measurement index were analyzed.Results The median sagittal surface of nasopalatine canal was funnel-shaped in 35 cases(34.3%),cylindrical in 33 cases(32.4%),hourglass in 26 cases(25.5%),and banana in 8 cases(7.8%).Among the variations of nasopalatine canal coronal morphology,there were 44 cases with single tube,2 cases with double-parallel tubes,and 56 cases with Y-shaped tubes,among which 46 cases had 2 Stenson holes and 10 cases had 3 or more.There were 43 nasopalatine canals with only 1 Stenson foramen and incisal foramen,and 1 Stenson foramen with 3incisal foramen.There were 48 cases with 2 stensons(46 cases with 1 incisor foramen and 2 cases with 2 incisor foramen).There were 10 cases with 3 or more Stenson foramen(3 Stenson foramen in 6 cases,4 Stenson foramen in 3cases,5 Stenson foramen in 1 case),all of which had only 1 incisor foramen.According to gender,nasopalatine duct diameter D1,diameter D3,length L1,lower labial bone plate thickness W2,nasopalatine angle(A)were statistically significant between the two groups(P<0.05),but not statistically significant between the two groups(P>0.05).According to age group,the median diameter of nasopalatine canal D2,the diameter of inferior mouth D3,and the thickness of labial bone plate W1 and W2 were statistically significant among the three groups(P<0.05),but the remaining differences were not statistically significant(P>0.05).Conclusion The anatomical variation of the nasopalatine canal is common in normal adults in Xiamen.The diameter and length of the u
