涂抹利多卡因乳膏联合利多卡因和罗哌卡因对肛门术后患者疼痛的临床效果分析

摘　　要：目的探讨与分析涂抹利多卡因乳膏联合利多卡因和罗哌卡因对治疗肛门术后患者疼痛的临床效果。方法选择2022年10月—2023年7月在汉中市中心医院进行手术的84例混合痔患者作为研究对象,根据随机掷骰子原则把84例患者分为研究组和对照组,每组42例。研究组给予利多卡因联合罗哌卡因局部注射麻醉与利多卡因乳膏涂抹镇痛麻醉,对照组单纯给予利多卡因联合罗哌卡因局部注射麻醉,比较两组患者的术后疼痛情况、生活质量、并发症发生率及麻醉满意度。结果研究组术后24 h、48 h、72 h的疼痛VAS评分明显低于对照组,差异有统计学意义(P<0.05)。研究组术后72 h的精神健康、生理功能、总体健康、躯体疼痛等生活质量评分都显著高于对照组,差异有统计学意义(P<0.05)。研究组术后72 h的尿潴留、肛门坠胀、恶心呕吐等并发症发生率显著低于对照组,差异有统计学意义(P<0.05)。研究组术后72 h的麻醉满意度显著高于对照组,差异有统计学意义(P<0.05)。结论涂抹利多卡因乳膏联合利多卡因和罗哌卡因能有效缓解肛门术后患者的疼痛,能降低患者的并发症发生率,提高患者的术后麻醉满意度与生活质量。Objective To explore and analysis the clinical effects of Lidocaine cream combined with Lidocaine and ropivacaine on postoperative pain in patients undergoing anal surgery.Methods From October 2022 to July 2023,84 cases of mixed hemorrhoids patients undergoing surgery in Hanzhong Central Hospital were selected as the study objects,and the other 84 cases of patients were divided into 42 cases in the research group and 42 cases in the control group accordedto the principle of random dice rolling.The research group were gave lidocaine and rovacaine and lidocaine cream,and the control group received lidocaine and rovacaine alone.Postoperative pain,quality of life,complication rate and satisfaction with anesthesia were recorded in both groups.Results The pain VAS score at 24 h,48 h and 72 h at the research group were significantly lower than the control group(P<0.05).The quality of life scores of mental health,physiological function,overall health,and physical pain in research group group at 72 hours after surgery were significantly higher than those in the control group(P<0.05).The incidence of complications,such as urinary retention,anal swelling,nausea and vomiting,was significantly lower than that of the control group,and the difference was significant(P<0.05).The satisfaction of anesthesia at 72h was significantly higher than the control group,statistically significant(P<0.05).Conclusion Applying Lidocaine cream combined with Lidocaine and ropivacaine can effectively relieve the pain of patients after anal surgery,reduce the incidence of complications and improve the satisfaction,quality of life scores of patients with anesthesia.
