摘 要:目的探究北京市朝阳区0~14岁儿童麻疹发病的影响因素,为降低儿童麻疹发病风险提供依据。方法采取病例对照研究的方法,选取朝阳区0~14岁儿童麻疹确诊病例156例,按1∶1进行配对,抽取156名健康儿童作为对照;运用条件logistic回归分析,对两组儿童的一般社会人口学资料、麻疹疫苗免疫史和其他可能的危险因素进行比较分析。结果多因素分析发现,在全部入选年龄组中,医院暴露是危险因素(OR=5.670、95%CI:2.933~10.959,P<0.001);有接种史是麻疹发病的保护因素(OR=0.043、95%CI:0.006~0.325,P<0.01);在0~7月龄儿童组,医院暴露(OR=10.398、95%CI:3.816~28.333,P<0.001)和出生在北京(OR=2.774、95%CI:1.025~7.506,P<0.05)是麻疹发病危险因素;8~17月龄儿童组有疫苗接种史是麻疹发病的保护因素(OR=0.043、95%CI:0.006~0.322,P<0.05);而18月龄~14岁儿童组未能纳入任何因素。结论要针对性地加强医院感染预防控制,加强麻疹疫苗的程序接种、漏种督查和及时补种复种工作。Objective To explore the incidence related factors of children measles at Chaoyang District in Beijing,and to provide the guideline for reducing the risk of measles incidence in children.Methods A case-control study was conducted to select 156 confirmed cases of measles in children aged 0-14 years at Chaoyang District as case group,and according to 1∶1 pairing,156 healthy children were selected as control group.Logistic regression analysis was used to compare the general social demographic data,immunization history of measles vaccine and other possible risk factors between two groups of children.Results The multivariate analysis found that,for all age groups,hospital exposure was a risk factor(OR=5.670,95%CI:2.933-10.959,P<0.001);the measles vaccination history was a protective factor(OR=0.043,95%CI:0.006-0.325,P<0.01);in 0-7 months old group of children,hospital exposure(OR=10.398,95%CI:3.816-28.333,P<0.001)and born in Beijing(OR=2.774,95%CI:1.025-7.506,P<0.05)were risk factors for measles;in 8-17 months old group,measles vaccination history was a protective factor(OR=0.043,95%CI:0.006-0.322,P<0.05);while there is not any factors included in the group of children aged 18 month to 14 years.Conclusion It is necessary to strengthen the prevention and control of hospital infection,strengthen the program of measles vaccine inoculation,the supervision of missing inoculation and the timely replanting work.
