摘 要:目的 分析研究血清微小RNA-155(miR-155)、白细胞介素-17(IL-17)与急性心肌梗死患者心肌酶谱及预后的相关性。方法 选取2021年1月至2022年4月皖北煤电集团总医院收治的急性心肌梗死(AMI)患者作为研究对象,将其命名为AMI组(n=129),另选同期在本院体检的健康人群为对照组(n=80)。比较两组血清miR-155、IL-17及心肌酶谱[肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)及乳酸脱氢酶(LDH)]水平;采用Pearson相关系数分析AMI患者血清miR-155、IL-17与心肌酶谱的相关性;在治疗结束后对患者随访1年,以随访期间发生心源性休克、心力衰竭、复发性心肌梗死以及患者出现死亡等事件为预后不良,根据多因素Logistic回归分析AMI患者预后不良的影响因素,并绘制ROC曲线分析血清miR-155、IL-17水平对AMI患者预后不良的预测价值。结果 AMI组的血清miR-155、IL-17、CK、CK-MB、LDH水平均高于对照组,差异有统计学意义(t=28.233,P<0.05);Pearson相关性分析显示,AMI患者血清miR-155、IL-17与CK、CK-MB、LDH呈正相关(均P<0.05);多因素Logistic回归分析显示,年龄>60岁(OR=2.083)、有高血压病史(OR=1.687)、有糖尿病史(OR=1.740)、miR-155水平高表达(OR=2.312)、IL-17水平升高(OR=1.966)、CK水平升高(OR=2.098)、CK-MB水平升高(OR=2.067)及LDH水平升高(OR=2.300)均是AMI患者预后不良的独立危险因素(P<0.05);ROC曲线分析显示,血清miR-155、IL-17水平及二者联合检测的曲线下面积(AUC)分别为0.930、0.865、0.964,联合检测优于单一检测(P<0.05)。结论 急性心肌梗死患者的血清miR-155、IL-17水平呈高表达,与心肌酶谱和预后情况密切相关。miR-155、IL-17可能成为急性心肌梗死患者预后不良的辅助预测指标。Objective To analyze the correlation between serum microRNA-155(miR-155)and interleukin-17(IL-17)and myocardial enzyme profile and prognosis in patients with acute myocardial infarc-tion.Methods Patients with acute myocardial infarction(AMI)admitted to the North Anhui Coal Power Group General Hospital from January 2021 to April 2022 were selected as the research subjects and referred to as the AMI group(n=129).Healthy individuals who underwent physical examinations at our hospital during the same period were selected as the control group(n=80).Serum miR-155,IL-17 and myocardial enzyme profiles[creatine kinase(CK),creatine kinase isoenzyme(CK-MB)and lactate dehydrogenase(LDH)]were compared between the two groups.Pearson correlation coefficient was used to analyze the correlation between serum miR-155,IL-17 and myocardial enzyme profiles in AMI patients.The patients were followed up for 1 year after the treatment,and the occurrence of cardiogenic shock,heart failure,recurrent myocardial infarction and death of patients during the follow-up period was considered as poor prognosis.The influencing factors of poor prognosis of AMI patients were analyzed according to multiple logistic regression.An ROC curve was drawn to analyze the predictive value of serum miR-155 and IL-17 levels for poor prognosis in AMI patients.Results The levels of serum miR-155,IL-17,CK,CK-MB and LDH in the AMI group were higher than those in the control group,with statistical significance(t=28.233,P<0.05).Pearson correlation analysis showed that serum miR-155 and IL-17 in AMI patients were positively correlated with CK,CK-MB and LDH(P<0.05).Multivariate logistic regression analysis showed that Age>60 years old(OR=2.083),history of hy-pertension(OR=1.687),history of diabetes(OR=1.740),high expression of miR-155(OR=2.312),elevated IL-17(OR=1.966),elevated CK(OR=2.098),elevated CK-MB(OR=2.067)and elevated LDH level(OR=2.300)were independent risk factors for poor prognosis in AMI patients(P<0.05).ROC curve analysis showed that the level of serum mi
