椎体成形术治疗老年骨质疏松性椎体骨折的临床疗效及对并发症的影响

摘　　要：目的 探讨椎体成形术治疗老年骨质疏松性椎体骨折的临床疗效及对并发症的影响。方法选取2020年4月—2023年4月邹城市人民医院收治的120例老年骨质疏松性椎体骨折患者作为研究对象,以简单抽签法分为对照组(n=60,内固定手术)和研究组(n=60,椎体成形术),比较两组治疗效果、脊柱功能评分及并发症发生情况。结果 研究组临床总有效率为98.33%,高于对照组的86.67%,差异有统计学意义(χ^(2)=4.324,P<0.05)。术前,两组脊柱影像学表现比较,差异无统计学意义(P>0.05);术后,研究组椎管侵占率、椎体压缩率以及Cobb角均优于对照组,差异有统计学意义(P均<0.05)。术前,两组脊柱功能评分比较,差异无统计学意义(P>0.05);术后,研究组Oswestry功效障碍指数以及视觉模拟评分均低于对照组,差异有统计学意义(P均<0.05)。研究组并发症发生率为1.67%,低于对照组的13.33%,差异有统计学意义(χ^(2)=4.324,P<0.05)。结论 椎体成形术治疗老年骨质疏松性椎体骨折的临床疗效明显,可以改善患者的脊柱功能,减少并发症的发生。Objective To investigate the clinical effect and complication of vertebroplasty in the treatment of senile osteoporotic vertebral fracture.Methods A total of 120 elderly patients with osteoporotic vertebral fracture admitted to Zoucheng People's Hospital from April 2020 to April 2023 were selected as the study objects.They were divided into control group(n=60,internal fixation surgery) and study group(n=60,verteboplasty) by simple drawing method,and the therapeutic effect,spinal function score and complications were compared between the two groups.Results The total clinical effective rate of the study group was 98.33%,higher than(86.67%) of the control group,and the difference was statistically significant(χ^(2)=4.324,P<0.05).Before surgery,there was no significant difference in spinal imaging findings between the two groups(P>0.05).The vertebral canal encroachment rate,vertebral compression rate and Cobb angle of the study group were better than those of the control group after surgery,and the differences were statistically significant(all P<0.05).Before operation,there was no significant difference in spinal function scores between the two groups(P>0.05).After surgery,the Oswestry Disability Index and Visual Analogue Scale scores of the study group were lower than those of the control group,and the differences were statistically significant(both P<0.05).The incidence of complications in the study group was 1.67%,lower than 13.33% in the control group,and the difference was statistically significant(χ^(2)=4.324,P<0.05).Conclusion Vertebroplasty in the treatment of senile osteoporotic vertebral fracture has obvious clinical effect,which can improve the spinal function of patients and reduce the occurrence of complications.
