百令胶囊对IgA肾病尿渗透压、尿蛋白及PCX、B7-1、β_(2)-MG、NAG水平的影响分析

摘　　要：目的 探讨百令胶囊联合缬沙坦对Ig A肾病(Immunoglobulin Anephropathy,Ig AN)尿渗透压、尿蛋白及尿足细胞标志蛋白(Podocalyxin,PCX)、尿足细胞B7-1分子(B7-1)、β_(2)-微球蛋白(β_(2)-microglobulin,β_(2)-MG)、N-乙酰-β-D氨基葡萄糖苷酶(N-Acetylglucosamine,NAG)水平的影响分析。方法 非随机选取2021年3月—2023年3月临朐县人民医院收治的74例Ig AN患者作为研究对象,按治疗方法分为治疗组与对照组,各37例。治疗组采用百令胶囊联合缬沙坦治疗,对照组单独口服缬沙坦治疗,均治疗2个月,比较两组尿渗透压、尿蛋白水平、肾小管损伤指标及临床疗效与不良反应发生率。结果 治疗2个月后,治疗组尿渗透压高于对照组,尿蛋白水平低于对照组,差异有统计学意义(P均<0.05);治疗组PCX、B7-1、β_(2)-MG及NAG水平低于对照组,差异有统计学意义(P均<0.05);治疗组的总体有效率(94.58%)高于对照组(78.38%),差异有统计学意义(χ^(2)=4.164,P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论 百令胶囊保护Ig AN的肾功能,有效减少尿蛋白,减轻近端肾小管损伤,不良反应少,值得临床借鉴。Objective To investigate the effects of Bailing capsule combined with valsartan on urinary osmotic pressure,urinary protein and urinary podocytoid marker podocalyxin (PCX),urinary podocytoid B7-1 molecule (B7-1),β_(2)-microglobulin (β_(2)-MG),N-Acetylglucosamine (NAG) levels in Ig A nephropathy (Ig AN).Methods A total of seventy-four Ig AN patients treated in Linqu County People's Hospital from March 2021 to March 2023 were nonrandomly selected as the study object,and divided into treatment group and control group with thirty-seven cases in each group by treatment method.The treatment group was treated with Bailing capsule combined with valsartan,and the control group was treated with oral valsartan alone.Both group treated for 2 months.Urinary osmotic pressure,urinary protein level,renal tubule injury index,clinical efficacy and incidence of adverse reactions were compared between the two groups.Results After 2 months of treatment,urinary osmolality in treatment group was higher than that in control group,and urinary protein level was lower than that in control group,the differences were statistically significant (both P<0.05).The levels of PCX,B7-1,β_(2)-MG and NAG in treatment group were lower than those in control group,and the differences were statistically significant (all P<0.05).The overall effective rate of the treatment group (94.58%) was higher than that of the control group (78.38%),the difference was statistically significant (χ^(2)=4.164,P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Bailing capsule can protect the renal function of Ig AN,effectively reduce urinary protein and alleviate proximal renal tubule injury,with few adverse reactions,which is worthy of clinical reference.
