摘 要:目的 分析尿微量白蛋白(Urinary Microalbumin,MALB)、血清胱抑素C(Serum Cystatin C,CysC)及β-2微球蛋白(Beta-2 Microglobulin,β_(2)-MG)检测在早期糖尿病肾病中的诊断价值。方法 非随机选取2022年7月—2023年6月新沂市人民医院进行体检的151名健康体检人员作为对照组,同时非随机选取151例单纯糖尿病患者作为观察A组,151例早期糖尿病肾病患者作为观察B组。所有研究对象均实施MALB、CysC及β_(2)-MG检测,对比MALB、CysC、β_(2)-MG检测及3种指标联合检测在早期糖尿病肾病中的诊断价值。结果 观察A组与观察组B组的CysC、β_(2)-MG及MALB、CysC均高于对照组,且观察B组高于观察A组,差异有统计学意义(P均<0.05)。MALB、CysC、β_(2)-MG联合诊断的特异度为95.70%,高于单一指标检测的83.77%、85.76%、83.11%,差异有统计学意义(χ^(2)=27.559,P<0.05)。联合诊断的灵敏度、准确率均高于单一指标检测,差异有统计学意义(P均<0.05)。结论 在早期糖尿病肾病的诊断中,采取MALB、CysC及β_(2)-MG水平联合检测诊断效能高于各单一检测的诊断效能。Objective To analyze the diagnostic value of urinary microalbumin(MALB),serum cystatin C(CysC) and β-2 microglobulin(β_(2)-MG) in early diabetic nephropathy.Methods A total of one hundred and fifty-one healthy physical examination personnel who underwent physical examination in Xinyi People's Hospital from July 2022 to June 2023 were non-randomly selected as the control group,while one hundred and fifty-one patients with simple diabetes were non-randomly selected as the observation group A and one hundred and fifty-one patients with early diabetic nephropathy were non-randomly selected as the observation group B.All subjects were tested by MALB,CysC and β_(2)-MG,and the diagnostic value of MALB,CysC,β_(2)-MG and the combined detection of the three indexes in early diabetic nephropathy were compared.Results The CysC,β_(2)-MG,MALB and CysC of observation group A and observation group B were higher than those of control group,and observation group B was higher than observation group A,the differences were statistically significant(all P<0.05).The specificity of MALB,CysC and β_(2)-MG combined diagnosis was 95.70%,which was higher than that of single index detection(83.77%,85.76% and 83.11%),and the difference was statistically significant(χ^(2)=27.559,P<0.05).The sensitivity and accuracy of combined diagnosis were higher than that of single index detection,and the differences were statistically significant(all P<0.05).Conclusion In the diagnosis of early diabetic nephropathy,the diagnostic efficacy of MALB,CysC and β_(2)-MG combined detection is higher than that of each single detection.
