腰麻-硬膜外联合麻醉对老年下肢骨折患者术后镇痛的影响效果分析

摘　　要：目的 分析对老年下肢骨折患者开展腰麻-硬膜外联合麻醉的术后镇痛效果。方法 目的选取江苏省盐城市东台梅氏中西医结合医院于2020年1月—2023年1月收治的80例老年下肢骨折手术患者为研究对象,依麻醉方案差异将其分为对照组(n=40,气管插管全身麻醉)和观察组(n=40,腰麻-硬膜外联合麻醉)。对比不同麻醉方式的麻醉效果。结果 观察组术后12、24、48 h疼痛程度评分低于对照组,观察组麻醉相关时间短于对照组,差异有统计学意义(P均<0.05)。观察组不良反应发生率(2.50%)低于对照组(20.00%),差异有统计学意义(χ^(2)=4.507,P<0.05)。结论 与全身麻醉相比,老年下肢骨折患者术中采用腰麻-硬膜外联合麻醉,术后镇痛效果更好,且术后不良反应少。Objective To analyze the postoperative analgesic effect of combined lumbar anesthesia and epidural anesthesia in elderly patients with lower limb fracture.Methods 80 elderly patients with lower extremity fracture surgery admitted to Dongtai Mei's Hospital of Integrated Chinese and Western Medicine from January 2020 to January 2023were purposefully selected as the study objects and were divided into control group(n=40,tracheal intubation general anesthesia) and observation group(n=40,combined lumbar anesthesia and epidural anesthesia) by the difference of anesthesia scheme.The anesthetic effects of different anesthesia methods were compared.Results At 12,24,48 h after surgery,the pain degree score of the observation group were lower those that of the control group,and the anesthesia related time of the observation group was shorter than that of the control group,the differences were statistically significant(all P<0.05).The incidence of adverse reactions in observation group(2.50%) was lower than that in control group(20.00%),and the difference was statistically significant(χ^(2)=4.507,P<0.05).Conclusion Compared with general anesthesia,the combination of lumbar anesthesia and epidural anesthesia in elderly patients with lower extremity fracture has better postoperative analgesia and less postoperative adverse reactions.
