摘  要:目的建立慢性阻塞性肺疾病(慢阻肺)患者的互联网+连续性智能管理模式并评估其应用价值。方法依托上海徐汇云医院平台建立“慢病空间”模块。患者通过此模块可记录症状,并可以给自己的医生留言。医生可以通过医生端,对患者进行病情随访及诊治方案调整。选取2021年5月—2023年1月在本院就诊的慢阻肺患者60例,按照随机数字表法将其分为试验组和常规组,每组30例,常规组采用常规门诊随访,试验组选择互联网+连续性智能模式进行随访,随访时间为6个月。评估试验组与常规组的干预前后肺功能指标、相关量表评分、6min步行试验(the six minute walking test,6MWT)距离。结果①6个月后,试验组患者第1s用力呼气容积(forced expiratory volume in the first second,FEV1)、用力肺活量(forced vital capacity,FVC)较常规组增加,常规组患者肺总量(vital capacity,VC)、残气量(residual volume,RV)降低,组间差异有统计学意义(P<0.05)。②6个月后,试验组患者慢阻肺评估测试(COPD assessment test,CAT)、焦虑自评量表(self—rating anxiety scale,SAS)、抑郁自评量表(self-rating depression scale,SDS)评分较常规组明显降低,生活质量量表(the 36-item short form survey,SF-36)评分较常规组升高,组间差异有统计学意义(P<0.05)。③试验组慢阻肺患者6MWT距离较常规组患者明显增加,组间差异有统计学意义(P<0.05)。结论采用互联网+连续性智能干预模式对慢阻肺患者进行干预,可有效改善患者肺功能状况,显著提升患者的生活质量。Objective To establish a Internet plus continuous intelligent management model for patients with chronic obstructive pulmonary disease(COPD)and evaluate its application value.Methods The paper established a"chronic disease space"module based on the Shanghai Xuhui Cloud Hospital platform,patients could record symptoms and leave messages to doctors through the module.Doctors could follow up patients and adjust diagnosis and treatment plans through the doctor's terminal.The paper chose 60 patients with COPD in our hospital from May 2021 to January 2023,and divided them into two groups according to random number table method,with 30 cases in each group.Routine group was followed up with routine outpatient method,and experimental group with the Internet plus continuous intelligent model,with followup of six months.Pulmonary function indicators,related scale scores,and distance of six minute walking test(6MWT)before and after intervention were evaluated between experimental group and control group.Results①After 6 months,forced expiratory volume in the first second FEV1 and forced vital capacity FVC in experimental group was higher than control group,while total lung capacity VC and residual volume RV was lower than control group,P<0.05.②After 6 months,COPD assessment test(CAT),Self Rating Anxiety Scale(SAS),and Self Rating Depression Scale(SDS)scores in experimental group were significantly lower than control group,while 36 Item Short Form Survey SF-36 scores were higher than control group,with P<0.05.③6MWT distance of patients with chronic obstructive pulmonary disease in experimental group was significantly less than control group,with P<0.05.Conclusion Internet plus continuous intelligent intervention mode can significantly improve lung function and life quality of COPD patients.
