摘　　要：目的探讨正常足月顺产新生儿脐静脉血pH值能否取代脐动脉血pH值来评估新生儿健康状况,及其影响因素。方法选取2020年1—12月符合纳入标准的150例产妇及新生儿为研究对象,用GEM Premier 3000进行血气分析,采用配对t检验,进行线性相关分析及回归分析。结果脐带动、静脉血平均pH值分别为(7.278±0.073)和(7.352±0.061),二者差异有统计学意义(P<0.001);脐带动、静血pH值在统计学上高度相关(r=0.782,P<0.05);脐带动、静脉血pH值、第二产程时长、产次、新生儿性别和需要吸氧分娩差异有统计学意义(P<0.05)。结论脐带动、静脉血pH值有显著差异,但是二者也显著相关;对于正常足月顺产新生儿,可以采集脐静脉血代替脐动脉血进行血气分析。Objective To explore whether pH value of umbilical vein blood can replace that of umbilical artery blood in evaluating health status of newborns during normal full-term natural delivery and its influencing factors.Methods The paper chose 150 pregnant women and newborns met inclusion criteria from January to December 2020 as study subjects,carried on blood gas analysis with GEM Premier 3000 and linear correlation analysis,regression analysis with paired t-test,Bland Altman plot.Results Average pH values of umbilical cord arterial and venous blood were(7.278±0.073)and(7.352±0.061)respectively,difference was statistically significant between them,(P<0.001).The pH value of umbilical cord blood was highly correlated statistically(r=0.782,P<0.05).The pH values of umbilical cord arterial and venous blood were significantly correlated with duration of the second stage of labor,parity,newborn gender,and need for oxygen inhalation(P<0.05).Conclusion There is significant difference in pH value of umbilical cord arterial and venous blood,but they are significantly correlated.Umbilical vein blood can be collected instead of umbilical artery blood to carry out blood gas analysis of normal full-term newborns born naturally.
