影响腰椎间盘突出症患者术后中长期疗效的相关因素分析

摘　　要：目的研究影响腰椎间盘突出症患者术后中长期临床疗效的相关因素。方法选择2020年7月—2022年7月天津北辰北门中医医院收治的100例腰椎间盘突出症患者,对所有患者进行持续半年的随访,根据随访半年后患者的康复情况,将其分为恢复良好组和恢复不良组,比较两组患者的一般资料,纳入影响因素,分析对腰椎间盘突出症患者术后中长期疗效的影响因素。结果半年后,恢复良好的患者共80例,占比为80%,恢复不良的患者共20例,占比为20%。恢复良好组和恢复不良组患者的病程、腰椎间盘突出类型、是否合并神经根粘连率比较,差异有统计学意义(P<0.05);恢复良好组和恢复不良组患者的性别、年龄、是否合并腰椎不稳、术后是否采取早期康复训练比较,差异无统计学意义(P>0.05)。通过多因素分析发现:病程超过6个月、突出型腰椎间盘、合并神经根粘连属于影响腰椎间盘突出症患者的中长期疗效的影响因素。结论腰椎间盘突出症患者术后中长期疗效受多种因素影响,为确保患者康复质量,临床实践中需基于具体情况,在随访期间精准施策,进行有效干预。此举对于提升患者术后中长期疗效具有重要意义,建议广泛推广实施。Objective To study relevant factors affecting mid to long-term clinical effect of patients with lumbar disc herniation after surgery.Methods The paper chose 100 patients with lumbar disc herniation admitted to Tianjin Beichen North Gate Traditional Chinese Medicine Hospital from July 2020 to July 2022,followed up for six months,and divided them into good recovery group and poor recovery group based on recovery after 6 months of follow-up.General information was compared between two groups,influencing factors were included to explore factors affecting longterm postoperative effect of patients with lumbar disc herniation.Results After half a year of followup,there were 80 cases with good recovery,accounting for 80%,and 20 cases with poor recovery,accounting for 20%,with significant difference in disease course,type of lumbar disc herniation,and presence of nerve root adhesion between two groups,(P<0.05).However,there was no significant difference in gender,age,presence of lumbar instability,and early postoperative rehabilitation training between two groups,(P>0.05).Multiple factor analysis found,factors of disease course exceeding 6 months,protruding lumbar intervertebral disc,combined nerve root adhesions affected mid to long-term effect of patients with lumbar disc herniation.Conclusion There are many factors affecting mid to long term postoperative effect of patients with lumbar disc herniation.Effective measures should be taken during follow-up based on specific influencing factor clinical practice,to improve mid to long term postoperative effect of patients,which is worthy of promo tion.
