摘　　要：目的评估分析社区医生健康促进项目对慢性病患者的效果。方法选取2022年5月—2023年6月社区医院收治的100例慢性病患者,采用盲法分为对照组和观察组,每组50例。其中,对照组接受传统的医疗服务,观察组接受社区医生健康促进项目的干预。对比两组患者健康行为情况、生活质量。结果观察组健康行为、生活质量各项评分均高于对照组,差异均有统计学意义(P<0.05)。结论社区医生健康促进项目对于慢性病患者而言,具有不可或缺的重要价值。该项目致力于为患者提供全方位、多层次的医疗服务与健康教育,确保患者得到及时、专业的医疗指导。通过实施综合治疗和关怀策略,社区医生健康促进项目能够有效地促进患者与医疗资源的紧密连接,为患者提供个性化的疾病管理方案。这不仅有助于患者更好地管理自身疾病,还能够有效促进健康行为的养成,从而提高患者的生活质量。Objective To evaluate and analyze effect of community doctor health promotion projects for chronic disease patients.Methods The paper chose 100 chronic disease patients in community hospitals from May 2022 to June 2023,and divided them into control group and observation group randomly,with 50 cases in each group.Control group was treated with traditional medical services,while observation group with interventions from community doctor health promotion projects.Health behavior and life quality was compared between two groups.Results Health behavior and life quality scores of observation group were higher than control group,difference was statistically significant(P<0.05).Conclusion Community doctor health promotion projects have important value for patients with chronic diseases,can provide comprehensive medical services and health education,provide psychological support and emotional nursing,promote connection between patients and medical resources.Based on comprehensive treatment and care strategies,community doctor health promotion projects can help chronic disease patients manage illnesses better,promote healthy behaviors,and improve life quality.
