摘 要:目的:探究人体免疫缺陷病毒(HIV)/获得性免疫缺陷综合征(AIDS)患者抗病毒治疗时间对CD4^(+)T淋巴细胞计数、HIV病毒载量的影响。方法:回顾性收集2018年4月—2023年4月新余市人民医院收治的200例HIV/AIDS患者的临床资料,根据确诊后抗病毒治疗时间的不同分为A组(治疗满1年,n=30)、B组(治疗满2年,n=26)、C组(治疗满3年,n=45)、D组(治疗满4年,n=99)。比较各组治疗后HIV病毒载量及CD4^(+)T淋巴细胞计数,分析抗病毒治疗时间与CD4^(+)T淋巴细胞计数、HIV病毒载量的关系,采用多元线性回归分析HIV/AIDS抗病毒治疗后CD4^(+)T淋巴细胞计数变化的影响因素。结果:D组CD4^(+)T淋巴细胞计数高于A组、B组、C组,且C组CD4^(+)T淋巴细胞计数高于A组、B组,B组CD4^(+)T淋巴细胞计数高于A组,差异均有统计学意义(P<0.05)。四组的HIV病毒载量比较,差异无统计学意义(P>0.05)。不同性别、基线CD4^(+)T淋巴细胞计数、WHO临床分期的HIV/AIDS患者抗病毒治疗后的CD4^(+)T淋巴细胞计数比较,差异均有统计学意义(P<0.05);多元线性回归分析显示,基线CD4^(+)T淋巴细胞计数、治疗时间为HIV/AIDS患者抗病毒治疗后CD4^(+)T淋巴细胞计数变化的影响因素(P<0.05)。结论:抗病毒治疗持续时间越长,HIV/AIDS患者的CD4^(+)T淋巴细胞计数改善效果越明显,基线CD4^(+)T淋巴细胞计数、治疗时间为HIV/AIDS患者抗病毒治疗后CD4^(+)T淋巴细胞计数变化的影响因素。Objective:To explore the effects of antiviral treatment time on CD4^(+)T lymphocyte count and HIV viral load in human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS)patients.Method:Clinical data of 200 HIV/AIDS patients admitted to Xinyu People's Hospital from April 2018 to April 2023 were retrospectively collected.According to the antiviral treatment time after diagnosis,they were divided into group A(treatment for 1 year,n=30),group B(treatment for 2 years,n=26),group C(treatment for 3 years,n=45)and group D(after 4 years of treatment,n=99).HIV viral load and CD4^(+)T lymphocyte count after treatment were compared among all groups,and the relationship between antiviral treatment time and CD4^(+)T lymphocyte count,HIV viral load was analyzed.Multiple linear regression was used to analyze the influencing factors of CD4^(+)T lymphocyte count changes after HIV/AIDS antiviral treatment.Result:The CD4^(+)T lymphocyte count of group D was higher than that of group A,group B and group C,and the CD4^(+)T lymphocyte count of group C was higher than that of group A and group B,and the CD4^(+)T lymphocyte count of group B was higher than that of group A,the differences were statistically significant(P<0.05).There was no significant difference in HIV viral load among the four groups(P>0.05).The CD4^(+)T lymphocyte counts of HIV/AIDS patients with different genders,baseline CD4^(+)T lymphocyte counts,and WHO clinical stages after antiviral treatment were compared,the differences were statistically significant(P<0.05).Multiple linear regression analysis showed that baseline CD4^(+)T lymphocyte count and treatment time were factors influencing the change of CD4^(+)T lymphocyte count in HIV/AIDS patients after antiviral treatment(P<0.05).Conclusion:The longer the duration of antiviral treatment,the more obvious the improvement effect of CD4^(+)T lymphocyte count in HIV/AIDS patients.Baseline CD4^(+)T lymphocyte count and treatment time are the influencing factors for the change of CD4^(+)T lymphocyte count
