布地奈德联合红霉素治疗百日咳患儿的效果观察

摘　　要：目的:探讨布地奈德联合红霉素治疗百日咳患儿的效果。方法:选取2022年4月—2024年3月赣州市妇幼保健院收治的96例百日咳患儿,以抽签法分为对照组、治疗组,各48例。对照组予红霉素治疗,治疗组予布地奈德联合红霉素治疗;比较两组治疗效果、症状改善时间、肺功能指标、炎症指标、免疫指标及不良反应发生情况。结果:治疗组总有效率高于对照组,痉挛性咳嗽、气促、肺部啰音、咳喘改善时间均早于对照组,差异均有统计学意义(P<0.05)。治疗2周后,治疗组呼吸频率(RR)、白细胞介素-8(IL-8)、白细胞介素-4(IL-4)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平均低于对照组,潮气量(TV)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平均高于对照组,差异均有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:百日咳患儿采用布地奈德联合红霉素治疗效果较好,可缩短症状改善时间,提高肺功能,降低炎症指标水平,改善免疫状况,且安全性好。Objective:To investigate the effect of Budesonide combined with Erythromycin in the treatment of children with pertussis.Method:A total of 96 children with pertussis treated in Ganzhou Maternal and Child Health Hospital from April 2022 to March 2024 were selected and divided into control group and treatment group by lottery,with 48 cases in each group.The control group was treated with Erythromycin,and the treatment group was treated with Budesonide combined with Erythromycin.The therapeutic effect,symptom improvement time,pulmonary function index,inflammatory index,immune index and occurrence of adverse reactions were compared between the two groups.Result:The total effective rate of the treatment group was higher than that of the control group,and the improvement time of spastic cough,shortness of breath,pulmonary rales,cough and panting were earlier than those of the control group,the differences were statistically significant(P<0.05).After 2 weeks of treatment,the levels of respiratory rate(RR),interleukin-8(IL-8),interleukin-4(IL-4),C reactive protein(CRP)and tumor necrosis factor-α(TNF-α)in the treatment group were lower than those in the control group,the levels of tidal volume(TV),immunoglobulin A(IgA),immunoglobulin M(IgM)and immunoglobulin G(IgG)in the treatment group were higher than those of control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:The treatment effect of Budesonide combined with Erythromycin in children with pertussis is better,which can shorten the time of symptom improvement,improve lung function,reduce inflammatory indicators,improve immune status,and has good safety.
