常规西药联合恒古骨伤愈合剂及活血补肾汤对激素性股骨头坏死患者的影响

摘　　要：目的:探讨常规西药联合恒古骨伤愈合剂及活血补肾汤治疗激素性股骨头坏死(SONFH)的效果。方法:选取2022年11月—2023年11月泰安市中医医院骨伤一科收治的87例SONFH患者,按随机数字表法分为对照组(n=44)及观察组(n=43)。对照组行常规西药治疗,观察组在对照组基础上加用恒古骨伤愈合剂及活血补肾汤治疗。比较两组髋关节功能、血液流变学指标、血脂指标、骨代谢指标及不良反应发生情况。结果:治疗后,观察组Harris髋关节评分(HHS)中各维度评分及总分均高于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组全血高切黏度(HBV)、全血低切黏度(LBV)、血浆黏度(PV)、红细胞压积(HCT)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)均低于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组骨钙素(BGP)、Ⅰ型前胶原氨基端前肽(PⅠNP)均高于对照组,抗酒石酸酸性磷酸酶(TRAP)低于对照组,差异均有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论:常规西药联合恒古骨伤愈合剂及活血补肾汤可改善SONFH患者血脂及血液流变学指标,加快髋关节功能恢复,且安全性良好。Objective:To investigate the effect of conventional western medicine combined with OSTEOKING and Huoxue Bushen Decoction in the treatment of steroid-induced osteonecrosis of the femoral head(SONFH).Method:A total of 87 SONFH patients admitted to the Department of Orthopedics and Trauma of Tai’an Traditional Chinese Medicine Hospital from November 2022 to November 2023 were selected and divided into control group(n=44)and observation group(n=43)according to random number table method.The control group was treated with conventional western medicine,and the observation group was treated with OSTEOKING combined with Huoxue Bushen Decoction on the basis of control group.Hip function,hemorheology,blood lipids,bone metabolism and adverse reactions were compared between the two groups.Result:After treatment,each dimension score and total score of the Harris hip score(HHS)in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,whole blood high tangential viscosity(HBV),whole blood low tangential viscosity(LBV),plasma viscosity(PV)and hematocrit(HCT),triglyceride(TG),total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)in observation group were lower than those in control group,the differences were statistically significant(P<0.05).After treatment,osteocalcin(BGP)and typeⅠprocollagen amino terminal propeptide(PⅠNP)in observation group were higher than those in control group,and tartrate-resistant acid phosphatase(TRAP)was lower than that in control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Conventional western medicine combined with OSTEOKING and Huoxue Bushen Decoction can improve blood lipids and hemorheology indexes in patients with SONFH,and accelerate the recovery of hip joint function with good safety.
