格列齐特缓释片联合二甲双胍对老年2型糖尿病患者胰岛功能和肾功能的影响

摘　　要：目的:探讨格列齐特缓释片联合二甲双胍对老年2型糖尿病(T2DM)患者胰岛功能和肾功能的影响。方法:选取2022年3月至2023年4月该院收治的68例老年T2DM患者进行前瞻性研究,按照随机数字表法将其分为研究组和对照组各34例。对照组口服盐酸二甲双胍片治疗,研究组在对照组基础上联合格列齐特缓释片治疗。比较两组治疗前后血糖指标[空腹血糖(FPG)、餐后2 h血糖(2hPG)]、胰岛素功能指标[空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)]、血清炎性因子[白三烯B4(LTB4)和趋化因子受体4(CXCR4)]、肾功能指标[胱抑素C(CysC)、血肌酐(SCr)、24 h尿蛋白总量(24UP)]水平,以及不良反应发生率。结果:治疗后,研究组FPG、2hPG、FINS、HOMA-IR、LTB4、SCr、CysC及24hUP水平均低于对照组,血清CXCR4水平高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:格列齐特缓释片联合二甲双胍治疗老年T2DM患者效果显著,能够降低血糖水平,减轻胰岛素抵抗,缓解机体微炎症状态,保护肾功能,效果优于单用二甲双胍治疗。Objective:To investigate effects of Gliclazide sustained-release tablets combined with Metformin on islet function and renal function in elderly patients with type 2 diabetes mellitus(T2DM).Methods:A prospective study was conducted on 68 elderly patients with T2DM admitted to the hospital from March 2022 to April 2023.According to the random number table method,they were divided into study group and control group,34 cases in each group.The control group was treated with Metformin hydrochloride tablets orally,while the study group was treated with Gliclazide sustained-release tablets on the basis of that of the control group.The levels of blood glucose[fasting blood glucose(FPG),2 h postprandial blood glucose(2hPG)],insulin function indexes[fasting insulin(FINS),insulin resistance index(HOMA-IR)],serum inflammatory factors[leukotriene B4(LTB4)and chemokine receptor 4(CXCR4)]and renal function indexes[cystatin C(CysC),serum creatinine(SCr),24 h urine protein(24UP)]before and after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:After the treatment,the levels of FPG,2hPG,FINS,HOMA-IR,LTB4,SCr,CysC and 24UP in the study group were lower than those in the control group,the level of serum CXCR4 was higher than that in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Gliclazide sustained-release tablets combined with Metformin in the treatment of the elderly patients with T2DM has a significant effect,which can reduce the blood glucose levels,reduce the insulin resistance,alleviate the body’s micro-inflammatory state,and protect the renal function.Moreover,it is superior to simple Metformin treatment.
