住院老年患者痴呆恐惧现状及影响因素研究

摘　　要：目的探讨住院老年患者痴呆恐惧现状及影响因素。方法采用一般资料问卷、痴呆恐惧量表、老年抑郁量表、自尊量表及社会支持评定量表对458例住院老年患者进行调查。结果住院老年患者痴呆恐惧得分54.00(41.00,66.00)分,老年抑郁得分4.00(3.00,7.00)分,社会支持得分28.00(24.00,32.00)分,自尊得分(27.94±4.47)分。多元线性回归分析显示,所患慢性病种类、个人月收入、身边有无痴呆患者、所患疾病是否会引起痴呆、自评健康状况及抑郁为住院老年患者痴呆恐惧的影响因素(均P<0.05)。结论住院老年患者痴呆恐惧处于中等水平,需重点关注慢性病多病共存、低收入、自评健康状况较差、痴呆暴露情况较多以及有抑郁症状的老年患者,采取积极的心理干预措施和健康教育,降低老年患者不必要的痴呆恐惧心理,提高其心理应对水平。Objective To investigate the present situation of dementia fear in hospitalized elderly patients and analyze its influencing factors.Methods A questionnaire survey was conducted among 458 hospitalized elderly patients using general information questionnaire,Fear of Demrntia Scale,the Geriatric Depression Scale 15,Social Support Rating Scale and Self-esteem Scale.Results The score of dementia fear among hospitalized elderly patients was 54.00(41.00,66.00),the depression score was 4.00(3.00,7.00),the score of social support was 28.00(24.00,32.00)and the score of self-esteem was(27.94±4.47).Multiple linear regression analysis showed that the presence of chronic diseases,personal monthly income,presence of dementia patients around,whether the disease can cause dementia,self-rated health status,and elderly depression were influencing factors of dementia fear among hospitalized elderly patients(all P<0.05).Conclusion Dementia fear among hospitalized elderly patients is at a moderate level.Priority should be given to elderly patients with multiple chronic diseases,low income,poor self-rated health status,more exposure to dementia,and depression symptoms.Positive psychological intervention measures and health education should be taken to reduce unnecessary dementia fear in the elderly and improve their psychological coping level.
