摘  要:目的探讨心源性猝死(SCD)患者窦房结组织超极化激活环核苷酸门控通道4(HCN4)和钙依赖性蛋白激酶Ⅱ(CaMKⅡ)蛋白表达情况及其法医学意义。方法收集2018年1月至2022年12月川北医学院司法鉴定中心尸检心脏标本200份,其中SCD100份(SCD组),非SCD100份(非SCD组)。两组均进行常规苏木精-伊红(HE)染色,采用免疫组织化学方法检测两组标本窦房结组织中HCN4和CAMKⅡ蛋白表达,图像处理软件计算目的蛋白的AOD值。结果SCD以成年男性为主且在死因中以冠心病占比最多。SCD组中HCN4的AOD值低于非SCD组,CAMKⅡ的AOD值高于非SCD组,差异有统计学意义(P<0.05)。结论窦房结中HCN4、CaMKⅡ异常表达可能是心源性猝死的机制,HCN4、CaMKⅡ有望成为心源性猝死的分子标记。Objective To investigate the expression of hyperpolarization activated current 4(HCN4)and Ca2+/calmodulin-dependent protein ki-naseⅡ(CAMKⅡ)in sinoatrial node tissue and its forensic significance in patients with sudden cardiac death(SCD).Methods A total of 200 autop-sy heart specimens were collected from the Judicial Identification Center of North Sichuan Medical College from January 2018 to December 2022,including 100 with SCD(SCD group)and 100 with non-SCD(non-SCD group).Routine hematoxylin and eosin staining(HE)staining was per-formed in both groups,the expression of HCN4 and CAMKⅡprotein in sinoatrial node tissues of the two groups was detected by immunohistochem-istry,the AOD value of the target protein was calculated by image processing software.Results SCD is dominated by adult males and coronary ath-erosclerotic heart disease accounts for the largest proportion of deaths.The AOD value of HCN4 in the SCD group was lower than that in the non-SCD group,and the AOD value of CAMKⅡwas higher than that in the non-SCD group,and the differences were statistically significant(P<0.05).Conclusion The abnormal expression of HCN4 and CAMKⅡin sinoatrial node may be one of the mechanisms of sudden cardiac death,HCN4 and CAMKⅡmay be the molecular markers of sudden cardiac death.
