摘　　要：目的探讨支气管镜下肺泡灌洗术治疗支气管肺炎并肺不张的临床疗效及对患儿C反应蛋白(CRP)及肺功能的影响。方法选取2019年1月至2021年12月赣州市妇幼保健院收治的90例支气管炎并肺不张患儿作为研究对象,按照摸球法分为对照组与观察组,各45例。对照组给予常规对症治疗,观察组在对照组基础上行支气管镜下肺泡灌洗术,比较两组临床疗效、CRP水平、肺功能及肺复张率。结果观察组治疗总有效率高于对照组(P<0.05);治疗后1个月,观察组CRP水平低于对照组(P<0.05);治疗后1个月,观察组用力肺活量(FVC)、第1秒用力呼气容积(FEV1)均大于对照组,FEV1/FVC高于对照组(P<0.05);观察组肺复张率高于对照组(P<0.05)。结论支气管镜下肺泡灌洗术治疗支气管炎并肺不张效果显著,能够有效降低CRP水平,改善患儿肺部功能,提高肺复张率,值得临床推广应用。Objective To explore the clinical effect of bronchoscopic alveolar lavage in the treatment of bronchopneumonia complicated with at-electasis and its influence on C-reactive protein(CRP)and lung function.Methods 90 children with bronchitis complicated with atelectasis admit-ted to Ganzhou Maternal and Child Health Hospital from January 2019 to December 2021 were selected as the research subjects,and they were divid-ed into the control group and the observation group according to the method of touching the ball,with 45 cases in each group.The control group was given conventional symptomatic treatment,while the observation group was given bronchoalveolar lavage on the basis of the control group,the clini-cal efficacy,CRP level,lung function and lung recruitment rate were compared between the two groups.Results The total effective rate in the obser-vation group was higher than that in the control group(P<0.05);1 month after treatment,the CRP level in the observation group was lower than that in the control group(P<0.05).1 month after treatment,the forced vital capacity(FVC)and forced expiratory volume in one second(FEV1)in the ob-servation group were higher than those in the control group,and FEV1/FVC was higher than that in the control group(P<0.05).The lung recruitment rate inthe observation group was higher than that in the control group(P<0.05).Conclusion Bronchoscopic alveolar lavage is effective in the treat-ment of bronchitis and atelectasis,it can effectively reduce the level of CRP,improve the lung function of children,and improve the lung recruitment rate,and is worthy of clinical promotion and application.
