纤支镜肺泡灌洗联合振动排痰与体位引流治疗重症支原体肺炎的临床效果分析

摘　　要：目的探讨纤支镜肺泡灌洗联合振动排痰与体位引流治疗重症支原体肺炎的临床效果。方法回顾性选取漳州正兴医院于2023年2月-2024年2月收治的86例重症支原体肺炎患儿的临床资料,根据治疗方式的不同分为对照组(n=43,纤支镜肺泡灌洗治疗)和观察组(n=43,联合振动排痰与体位引流治疗),对比两组的临床疗效、住院时间及症状消失时间、实验室指标、血气指标及心率水平。结果观察组的临床治疗总有效率为97.67%,高于对照组的81.40%,差异有统计学意义(χ^(2)=4.468,P<0.05)。治疗后,观察组的住院时间及症状消失时间短于对照组,实验室指标低于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组动脉血氧分压水平高于对照组,动脉血二氧化碳分压、心率水平低于对照组,差异有统计学意义(P均<0.05)。结论纤支镜肺泡灌洗联合振动排痰与体位引流治疗重症支原体肺炎的临床效果突出,可以缩短患者的住院时间、退热时间、咳嗽好转时间,改善血气分析指标以及心率,同时降低实验室指标。Objective To explore the clinical effect of bronchoscopic alveolar lavage combined with vibration expecto-ration and postural drainage in the treatment of severe mycoplasma pneumonia.Methods The clinical data of 86 chil-dren with severe mycoplasma pneumonia admitted to Zhangzhou Zhengxing Hospital from February 2023 to February 2024 were retrospectively selected.According to the different treatment methods,they were divided into control group(n=43,bronchoscopic alveolar lavage treatment)and observation group(n=43,combined with vibration sputum drain-age and postural drainage treatment).The clinical efficacy,hospitalization time and symptom disappearance time,labo-ratory index level,blood gas index and heart rate level of the two groups were compared.Results The total effective rate of clinical treatment in the observation group was 97.67%,which was higher than 81.40%in the control group,the difference was statistically significant(χ^(2)=4.468,P<0.05).After treatment,the hospitalization time and symptom disappearance time of the observation group were shorter than those of the control group,and the laboratory indexes were lower than those of the control group,the differences were statistically significant(all P<0.05).After treatment,the level of arterial partial pressure of oxygen in the observation group was higher than that in the control group,and the levels of arterial partial pressure of carbon dioxide and heart rate were lower than those in the control group,the differences were statistically significant(all P<0.05).Conclusion The clinical effect of fiberoptic bronchoscopy alveo-lar lavage combined with vibration sputum drainage in the treatment of severe mycoplasma pneumonia is outstanding.It can shorten the hospitalization time,antipyretic time,cough improvement time,improve blood gas analysis index and heart rate of patients,and reduce laboratory indicators.
