德清县县域医共体下家庭医生和家庭病房发展的PEST分析及对策研究

摘　　要：随着人口老龄化的加剧,老年性疾病发病率逐渐增加,传统医疗模式无法满足老年性疾病治疗需求。家庭医生签约服务和县域医共体改革属于分级诊疗中的两项重要措施。家庭医生能为患者提供定期巡诊、咨询服务以及长期的医疗管理和治疗,促进了医养结合,可有效缓解医疗资源供需矛盾。县域医共体指以县级医院为龙头,整合县乡医疗卫生资源,实施集团化运营管理,着力改革完善县级医院、乡镇卫生院(社区卫生服务中心)的管理体制和运行机制,形成服务共同体、责任共同体、利益共同体、管理共同体,促进县域内医疗卫生资源合理配置、医共体内人员正常流动、基层医疗服务能力明显提升、就医秩序合理规范。该文应用PEST分析法,从政治因素、经济因素、社会因素和技术因素4个方面分析德清县县域医共体下家庭医生和家庭病房发展现状,提出优化建议和发展对策,为促进分级诊疗和医养结合提供参考。With the aging of the population,the incidence of senile diseases is gradually increasing,and the traditional medical model cannot meet the treatment needs of senile diseases.The contract service of family doctors and the reform of county medical community are two important measures in hierarchical diagnosis and treatment.Family doctors can provide patients with regular visits,consultation services and long-term medical management and treatment,promote the combination of medical and nursing care,and effectively alleviate the contradiction between supply and demand of medical resources.The county medical community refers to the county-level hospital as the leader,the integration of county and township medical and health resources,the implementation of group operation and management,and the reform and improvement of the management system and operation mechanism of county hospitals and township health centers(community health service centers).To form a community of service,responsibility,interests and management,and promote the rational allocation of medical and health resources within the county,the normal flow of medical personnel in the medical community,the significant improvement of primary medical service capacity,and the rational and standardized order of medical treatment.In this paper,PEST analysis was applied to analyze the development status of family doctors and family wards in Deqing County from four aspects:political factors,economic factors,social factors and technical factors,and optimization suggestions and development countermeasures were put forward to provide reference for promoting graded diagnosis and treatment and combining medical and nursing care.
