摘 要:目的 观察银杏叶提取物在高血压脑出血(HICH)患者微创穿刺术后的应用效果。方法 选取2022年7月—2023年6月于湖北省麻城市人民医院接受微创穿刺术治疗的HICH患者78例,以随机数字表法分成研究组和对照组,每组39例。研究组给予银杏叶提取物联合常规治疗,对照组给予常规治疗,2组患者均治疗14 d。比较2组患者临床疗效,治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、血脑屏障功能指标[血清白蛋白(S-Alb)、脑脊液白蛋白(CSF-Alb)、血脑屏障指数]、脑源性神经营养因子(BDNF)、神经元特异性烯醇化酶(NSE)、基质金属蛋白酶-9(MMP-9)水平及不良反应。结果 研究组患者总有效率为89.74%,高于对照组的71.79%(χ^(2)=4.044,P=0.044)。治疗14 d后,2组患者NIHSS评分均低于治疗前,且研究组低于对照组(P<0.01);2组患者CSF-Alb及血脑屏障指数均低于治疗前,S-Alb水平均高于治疗前,且研究组低/高于对照组(P<0.05或P<0.01);2组患者BDNF水平均高于治疗前,MMP-9、NSE水平均低于治疗前,且研究组高/低于对照组(P<0.01)。研究组与对照组患者不良反应总发生率比较差异无统计学意义(7.69%vs.2.56%,χ^(2)=1.054,P=0.305)。结论 银杏叶提取物用于HICH患者微创穿刺术后,可提升临床疗效,降低神经功能缺损程度,对患者血脑屏障功能、相关血清指标有显著改善作用,安全性较高。Objective To observe the application effect of Ginkgo biloba extract after minimally invasive puncture surgery in patients with hypertensive intracerebral hemorrhage(HICH).Methods A total of 78 HICH patients who underwent minimally invasive puncture treatment at People′s Hospital of Macheng City in Hubei Province from July 2022 to June 2023 were selected and randomly divided into study group and control group,with 39 cases in each group,using a random number table method.The study group received a combination of Ginkgo biloba extract and conventional treatment,while the control group received conventional treatment.Both groups of patients were treated for 14 days.The clinical efficacy,National Institutes of Health Stroke Scale(NIHSS) score,blood-brain barrier function indicators[serum albumin(S-Alb),cerebrospinal fluid albumin(CSF-Alb),blood-brain barrier index],brain-derived neurotrophic factor(BDNF),neuron specific enolase(NSE),matrix metalloproteinase-9(MMP-9) levels,and adverse reactions before and after treatment between the two groups.Results The total effective rate of the study group was 89.74%,higher than that of the control group(71.79%)(χ^(2)=4.044,P=0.044).After 14 days of treatment,the NIHSS score of 2 groups were lower than before treatment,and the study group was lower than the control group(P<0.01).CSF-Alb and blood-brain barrier index in 2 groups were lower than before treatment,and S-Alb levels were higher than before treatment,and the study group was lower/higher than the control group(P<0.05 or P<0.01).BDNF levels in 2 groups were higher than before treatment,MMP-9 and NSE levels were lower than before treatment,and the study group was higher/lower than the control group(P<0.01).There was no significant difference in the incidence of adverse reactions between the study group and the control group(7.69% vs.2.56%,χ^(2)=1.054,P=0.305).Conclusion After using ginkgo biloba extract for minimally invasive puncture in patients with HICH,it can improve clinical efficacy,reduce the degree of neurolo
