血府逐瘀胶囊对胸椎骨折内固定术后患者疼痛及血清炎性因子的影响

摘　　要：目的观察血府逐瘀胶囊对胸椎骨折内固定术后患者疼痛及血清炎性因子的影响。方法选取2022年12月—2023年10月绵竹市人民医院收治的胸椎骨折内固定术后患者80例,按随机数字表法分为观察组和对照组,各40例。对照组术后行常规西药治疗,观察组在对照组基础上给予血府逐瘀胶囊治疗。2组均治疗1个月,比较2组治疗前及治疗3 d后炎性因子[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-1(IL-1)、白介素-6(IL-6)]水平,治疗前及治疗1个月后疼痛程度[视觉模拟评分法(VAS)评分]、功能障碍情况[Oswestry功能障碍指数(ODI)]、血液流变学指标、生活质量[生活质量综合评定问卷(GQOLI-74)评分]。结果治疗3 d后,2组CRP、TNF-α、IL-1、IL-6水平均低于治疗前,且观察组低于对照组(P<0.01)。治疗1个月后,2组VAS评分及ODI均低于治疗前,且观察组低于对照组(P<0.01);2组全血低切黏度、全血高切黏度、红细胞比容均低于治疗前,且观察组低于对照组(P<0.01);2组GQOLI-74各维度评分均高于治疗前,且观察组高于对照组(P<0.01)。结论血府逐瘀胶囊可缓解胸椎骨折内固定术后患者疼痛,加速患者胸椎功能恢复,减轻炎性反应,改善血液流变学,提高生活质量。Objective To observe the effects of Xuefu Zhuyu capsule on pain and serum inflammatory factors in patients with thoracic vertebra fracture after internal fixation.Methods A total of 80 patients with thoracic vertebra fracture treated in Mianzhu People′s Hospital from December 2022 to October 2023 after internal fixation were selected and divided into observation group and control group according to random number table method,with 40 patients in each group.The control group was treated with conventional western medicine after operation,and the observation group was treated with Xuefu Zhuyu capsule on the basis of the control group.The two groups were treated for 1 month.The levels of inflammatory factors(CRP,TNF-α,IL-1,IL-6)before treatment and 3 days after treatment,pain degree(VAS score),dysfunction(ODI),hemorheology index,quality of life(GQOLI-74 score)before treatment and 1 month after treatment were compared between the two groups.Results After 3 days of treatment,the levels of CRP,TNF-α,IL-1 and IL-6 in two groups were lower than before treatment,and the observation group were lower than the control group(P<0.01).After 1 month of treatment,VAS score and ODI in two groups were lower than before treatment,and observation group were lower than control group(P<0.01).Whole blood low tangential viscosity,whole blood high tangential viscosity and erythrocyte specific volume in two groups were lower than before treatment,and observation group were lower than control group(P<0.01).The scores of GQOLI-74 in all dimensions in two groups were higher than before treatment,and the observation group were higher than the control group(P<0.01).Conclusion Xuefu Zhuyu capsule can relieve the pain of patients after internal fixation of thoracic vertebra fracture,accelerate the recovery of thoracic vertebra function,reduce inflammatory response,improve hemorheology and improve the quality of life.
