产前超声联合血清miR-148b、miR-375检测在胎儿先天性心脏病诊断中的应用价值

摘　　要：目的 探讨微小RNA(miR)-148b、miR-375在胎儿先天性心脏病孕妇血清中的水平,及其联合产前超声在胎儿先天性心脏病诊断中的价值。方法 收集2019年1月至2023年1月进行产前筛查的疑似胎儿先天性心脏病的106例孕妇作为研究对象,根据产后随访结果将其分为病例组(先天性心脏病,n=80)和对照组(均无先天性心脏病,n=26)。比较2组血清miR-148b、miR-375水平;ROC曲线分析血清miR-148b、miR-375对胎儿先天性心脏病的诊断效能;四表格法分析产前超声联合血清miR-148b、miR-375水平对胎儿先天性心脏病的诊断效能。结果 与对照组比较,病例组血清miR-148b水平显著降低,血清miR-375水平显著升高(P<0.05)。产前超声联合血清miR-148b、miR-375诊断胎儿先天性心脏病的准确率为93.40%,敏感性为92.50%,特异性为96.15%。其中,三项联合诊断的敏感性高于血清miR-148b、miR-375单独诊断(P<0.05),三项联合诊断的准确率和特异性高于产前超声、血清miR-148b、miR-375单独诊断(P<0.05)。结论 胎儿先天性心脏病的孕妇血清miR-148b水平显著降低,血清miR-375水平显著升高,产前超声联合血清miR-148b、miR-375对胎儿先天性心脏病具有较高的诊断价值。Objective To investigate the value of serum miR-148b and miR-375 levels combined with prenatal ultrasound in the diagnosis of fetal congenital heart disease(FCHD) by the measurement of microRNA(miR)-148b and miR-375 levels.Methods Totally 106 pregnant women with suspected FCHD who underwent prenatal screening in our hospital from January 2019 to January 2023 were enrolled.They were assigned into a case group(FCHD,n=80) and a control group(non-FCHD,n=26) based on postpartum follow-up.The levels of serum miR-148b and miR-375 were compared between the two groups.The receiver operating characteristic(ROC) curve was plotted to analyze the diagnostic efficacy of serum miR-148b and miR-375 for FCHD.The four grid table was applied to analyze the diagnostic efficacy of prenatal ultrasound combined with serum miR-148b and miR-375 levels for FCHD.Results The serum miR-148b level in the case group was significantly decreased than that in the control group,while the serum miR-375 level was significantly increased(P<0.05).The accuracy,sensitivity,and specificity of prenatal ultrasound combined with serum miR-148b and miR-375 in diagnosing FCHD were 93.40%,92.50%,and 96.15%,respectively.The sensitivity of the combined diagnosis was higher than that of serum miR-148b and miR-375 alone(P<0.05),the accuracy and specificity of the combined diagnosis were higher than those of prenatal ultrasound,serum miR-148b,and miR-375 alone(P<0.05).Conclusion FCHD have significantly reduced serum miR-148b level and increased serum miR-375 level.Prenatal ultrasound combined with serum miR-148b and miR-375 has high diagnostic value for FCHD.
