摘  要:目的 研究短时间内血糖浓度波动对NT-proBNP水平的影响。方法 选取2019年9~10月进行OGTT试验的住院患者32例。分别检测每位患者在空腹时、OGTT试验开始后30 min、60 min、120 min和180 min时血浆NT-proBNP、血糖、胰岛素和C-肽的水平。通过Pearson和Spearman相关分析探究个体NT-proBNP水平与血糖浓度的相关性,应用单因素重复测量方差分析和广义线性混合模型进一步分析检测时间、血糖、胰岛素和C-肽对NT-proBNP水平的影响。结果 相关分析表明,在32例患者中,只有5例患者的NT-proBNP水平与血糖浓度有相关性(P<0.05)。然而,每例患者NT-proBNP水平与血糖浓度的相关性趋势和最佳拟合曲线各不相同。单因素重复测量方差分析表明,不同时间和不同血糖浓度下NT-proBNP水平的差异无统计学意义[F(1.36,40.87)=1.23,P=0.29,η2=0.04]。广义线性混合模型表明NT-proBNP水平不受检测时间、血糖、胰岛素和C-肽浓度的影响(P>0.05)。结论 急性血糖浓度波动不影响NT-proBNP水平从而不影响NT-proBNP对心功能的评估。Objective To study the impact of acute blood glucose fluctuation on the level of N-terminal pro-brain natriuretic peptide(NT-proBNP).Methods Totally 32 inpatients undergoing oral glucose tolerance tests(OGTT) from September to October 2019 were randomly selected.Plasma NT-proBNP,blood glucose,insulin and C-peptide levels were measured at the fasting state,and 30-minute,60-minute,120-minute and 180-minute following OGTT.The correlation between individual NT-proBNP level and blood glucose concentration was assessed by Pearson's and Spearman's correlation coefficients.The one-way analysis of variance(ANOVA) and generalized linear mixed model(GLMM) were applied to further analyze the effects of the detection time of blood glucose,blood glucose level,insulin and C-peptide on NT-proBNP level.Results Correlation analysis showed that a significant correlation between NT proBNP level and blood glucose concentration was only found in 5 cases out of 32 patients(P<0.05).However,the correlation trends and best fit curves between NT proBNP level and blood glucose concentration varied in all the patients.One-way ANOVA did not identify a significant difference in NT proBNP level at different time points and different blood glucose concentrations(F [1.36,40.87] = 1.23,P=0.29,η~2=0.04).GLMM showed that NT-pro-BNP level was not impacted by the detection time of blood glucose,blood glucose level,insulin and C-peptide(P>0.05).Conclusion Acute blood glucose fluctuation does not impact the level of NT-proBNP,and thereby it does not impact the evaluation on cardiac function.
