微型钢板内固定术治疗跖跗关节复合体损伤的临床效果研究

摘　　要：目的探讨微型钢板内固定术治疗跖跗关节复合体损伤的临床效果。方法选择112例跖跗关节复合体损伤患者为研究对象,以随机数字表法分对照组(56例)与观察组(56例)。对照组应用克氏针内固定术治疗,观察组使用微型钢板内固定术治疗。比较两组的手术时间、骨折愈合时间、并发症发生情况、手术前后疼痛评分及功能恢复情况。结果观察组的手术时间(75.58±9.74)min长于对照组的(59.36±8.52)min,骨折愈合时间(92.24±11.39)d短于对照组的(103.39±15.58)d(P<0.05);观察组并发症总发生率为5.36%,低于对照组的21.43%,组间差异有统计学意义(P<0.05);观察组术后1周和术后3个月的视觉模拟评分法(VAS)评分分别为(4.87±1.39)、(1.02±0.34)分,均低于对照组的(5.92±1.27)、(1.39±0.54)分;观察组功能恢复优良率为78.57%,高于对照组的55.36%,组间差异有统计学意义(P<0.05)。结论微型钢板内固定术治疗跖跗关节复合体损伤可缩短骨折愈合时间,减少手术并发症发生风险,减轻术后疼痛程度,促进活动功能恢复,虽然延长了手术时间,但仍是值得推荐的手术方案。Objective To explore the clinical effect of mini-plate internal fixation for the treatment of injury to the tarsometatarsal joint complex.Methods 112 patients with injury to the tarsometatarsal joint complex were selected as the study subjects.They were randomly divided into a control group(56 cases)and an observation group(56 cases)by random number table method.The control group was given Kirschner wire internal fixation,while the observation group was given mini-plate internal fixation.The surgical time,fracture healing time,complications,pain score before and after surgery and functional recovery were compared between the two groups.Results In the observation group(P<0.05),the surgical time was(75.58±9.74)min,which was longer than(59.36±8.52)min in the control group;the healing time of fracture was(92.24±11.39)d,which was shorter than(103.39±15.58)d in the control group;the total complication rate was 5.36%,which was lower than 21.43%in the control group.The difference between groups was statistically significant(P<0.05).The visual analogue scale(VAS)scores of the observation group were(4.87±1.39)and(1.02±0.34)points at 1 week and 3 months after surgery,which were lower than(5.92±1.27)and(1.39±0.54)points of the control group;the excellent rate of functional recovery of the observation group was 78.57%,which was higher than 55.36%of the control group;the difference was statistically significant(P<0.05).Conclusion Mini-plate internal fixation for the treatment of injury to the tarsometatarsal joint complex can shorten the fracture healing time,reduce the risk of surgical complications,reduce the degree of postoperative pain,and promote the recovery of activity function.Although the surgical time is extended,it is still a recommended surgical program.
