腹膜外腹腔镜腹股沟疝修补术治疗老年腹股沟疝的效果及对患者术后并发症的影响研究

摘　　要：目的探究老年腹股沟疝治疗期间予以腹膜外腹腔镜腹股沟疝修补术的疗效及并发症情况。方法选取老年腹股沟疝患者82例参与分析,将入组患者以计算机表法分为对照组及研究组,每组41例。对照组给予常规治疗,研究组给予腹膜外腹腔镜腹股沟疝修补术治疗。对比两组患者疼痛评分、临床相关指标,治疗效果以及并发症发生率。结果研究组术后24 h、术后48 h、术后72 h、术后7 d疼痛评分分别为(6.21±0.42)、(4.25±0.21)、(2.57±0.37)、(1.03±0.24)分,对比对照组的(6.98±0.36)、(4.86±0.19)、(3.15±0.24)、(1.59±0.17)分较低,差异有统计学意义(P<0.05)。研究组手术时间(40.85±5.86)min、下床时间(3.85±0.52)d、住院时间(5.86±1.41)d短于对照组的(65.29±8.84)min、(5.24±0.65)d、(8.89±1.35)d,出血量(21.86±5.85)ml少于对照组的(40.56±4.35)ml,差异有统计学意义(P<0.05)。研究组治疗总有效率(97.56%)对比对照组(80.49%)更高,差异有统计学意义(P<0.05)。研究组并发症发生率(4.88%)对比对照组(21.95%)更低,差异有统计学意义(P<0.05)。结论相比常规治疗方案,临床采用腹膜外腹腔镜腹股沟疝修补术治疗老年腹股沟疝患者效果优异,能减少患者术后并发症,提高疾病预后效果,缓解患者疼痛情况,使患者尽早恢复健康,且能够面向基层医疗机构进行推广,值得临床广泛推荐使用。Objective To explore the efficacy of extraperitoneal laparoscopic hernia repair in the treatment of inguinal hernia in the elderly and its impact on postoperative complications in patients.Methods 82 elderly patients with inguinal hernia were selected for analysis.The enrolled patients were divided into a control group and a study group using computer table method,each with 41 patients.The control group received conventional treatment,and the study group received extraperitoneal laparoscopic hernia repair.The pain scores and clinical related indicators,treatment effect and incidence of complications of the two groups of patients were compared.Results The pain scores of the study group were(6.21±0.42),(4.25±0.21),(2.57±0.37)and(1.03±0.24)points at 24 h,48 h,72 h and 7 d after surgery,which were lower than(6.98±0.36),(4.86±0.19),(3.15±0.24)and(1.59±0.17)points of the control group,and the difference was statistically significant(P<0.05).The surgery time of the study group was(40.85±5.86)min,the time to get out of bed was(3.85±0.52)d and the hospitalization time was(5.86±1.41)d,which were shorter than(65.29±8.84)min,(5.24±0.65)d and(8.89±1.35)d of the control group;the bleeding volume in the study group was(21.86±5.85)ml,which was less than(40.56±4.35)ml in the control group;the difference was statistically significant(P<0.05).The total effective rate of the study group(97.56%)was higher than that of the control group(80.49%),and the difference was statistically significant(P<0.05).The complication rate of the study group(4.88%)was lower than that of the control group(21.95%),and the difference was statistically significant(P<0.05).Conclusion Compared with conventional treatment options,the clinical use of extraperitoneal laparoscopic hernia repair for elderly patients with inguinal hernia has excellent results.It can reduce the occurrence of postoperative complications,improve disease prognosis,alleviate patient pain,and enable patients to recover their health as soon as possible.It can be promoted
