摘　　要：目的探究高尿酸血症对老年2型糖尿病患者胰岛素抵抗和尿蛋白水平的影响.方法选取2021年8月-2023年10月韶关市曲江区人民医院收治的140例老年2型糖尿病患者,按照是否合并高尿酸血症分成两组,研究组为合并高尿酸血症的50例患者,对照组为未合并高尿酸血症的90例患者.结果两组患者的FPG、HbAlc、TG、TC、LDL-C、SCr和收缩压、舒张压水平无明显差异性(P>0.05),研究组的FINS、BUN、BUA、PRO、ALB水平更高(P<0.05),研究组的HDL-C水平更低(P<0.05);研究组的胰岛素抵抗水平更高(P<0.05);研究分析表明,高尿酸血症与老年2型糖尿病患者的ALB、FINS、HOMA-IR、HOMA-β指标呈正相关性(P<0.05).结论合并高尿酸血症老年2型糖尿病患者机体内的胰岛素抵抗和尿蛋白水平更高,与上述指标存在紧密的正相关性,为临床诊断与治疗老年2型糖尿病提供重要的参考依据.Objective To explore the effect of hyperuricemia on insulin resistance and urinary protein level in elderly patients with type 2 diabetes.Method 140 elderly patients with type 2 diabetes admitted to our hospital from August 2021 to October 2023 were selected and divided into two groups according to whether hyperuricemia was combined.The study group was 50 patients with hyperuricemia,and the control group was 90 patients without hyperuricemia.Result There was no significant difference in FPG,HbAlc,TG,TC,LDL-C,SCr,systolic and diastolic blood pressure levels between the two groups of patients(P>0.05).The study group had higher levels of FINS,BUN,BUA,PRO,and ALB(P<0.05),while the study group had lower levels of HDL-C(P<0.05);the insulin resistance level of the study group was higher(P<0.05);research analysis shows that ALB,FINS,HOMA-IR,HOMA-βin hyperuricemia and elderly patients with type 2 diabetes the indicators show a positive correlation(P<0.05).Conclusion The level of insulin resistance and urinary protein in the elderly patients with type 2 diabetes combined with hyperuricemia is higher,and there is a close positive correlation with the above indicators,which provides an important reference for the subsequent clinical diagnosis and treatment of elderly type 2 diabetes.
