N-Trap输尿管硬镜激光碎石术治疗输尿管上段结石的临床效果及对应激反应指标的影响

摘　　要：目的探究与分析拦截网篮(N-Trap)输尿管硬镜激光碎石术治疗输尿管上段结石的临床效果及对应激反应指标的影响.方法采取随机数字表法对苏州市吴江区第五人民医院自2021年2月-2024年1月收治的90例输尿管上段结石患者分组,每组各45例,对照组采用输尿管硬镜激光碎石术治疗,观察组采用N-Trap输尿管硬镜激光碎石术治疗,对比两组围术期指标、手术前后应激反应指标,同时对比两组手术相关并发症.结果观察组与对照组相比,术中结石上移率较低、手术费用较高、首次结石清除率较高,差异有统计学意义(P<0.05).观察组与对照组相比,手术时间、术后无意义残石率、术后住院时间、近期以及远期并发症发生率,差异无统计学意义(P>0.05).两组患者手术后与手术前比较,CRP、IL-6及Cor水平较高,差异有统计学意义(P<0.05).两组手术后的CRP、IL-6及Cor水平比较,差异无统计学意义(P>0.05).结论N-Trap输尿管硬镜激光碎石术治疗输尿管上段结石时具有更好的围术期指标,且并不会对应激反应指标造成不良刺激,同时该操作也并不会增加近远期并发症发生率,安全性较高.Objective To explore and analyze the clinical effect of N-Trap ureteroscopic laser lithotripsy in the treatment of upper ureteral calculi and its influence on stress response indexes.Methods Ninety cases of upper ureteral calculi admitted to our hospital from February 2021 to January 2024 were divided into 45 cases in each group by random number table method.The control group was treated with ureteroscopic laser lithotripsy,while the observation group was treated with N-Trap ureteroscopic laser lithotripsy.Perioperative indexes and stress response indexes before and after surgery were compared between the two groups.At the same time,the surgical complications were compared between the two groups.Results Compared with the control group,the rate of intraoperative stone ascent was lower,the operation cost was higher,and the first stone clearance rate was higher in the observation group,the difference was statistically significant(P<0.05).There was no significant difference between the observation group and the control group in operation time,postoperative meaningless stone residue,postoperative hospital stay,and incidence of short-term and long-term complications(P>0.05).The levels of CRP,IL-6 and Cor were higher after surgery than before surgery,and the differences were statistically significant(P<0.05).There was no significant difference in CRP,IL-6 and Cor levels between the two groups after operation(P>0.05).Conclusion N-Trap ureteroscopic laser lithotripsy in the treatment of upper ureteral calculi has better perioperative indexes,and does not cause adverse stimulation to stress response indexes.At the same time,this operation does not increase the incidence of short-term and long-term complications,and has high safety.
