摘　　要：目的观察草酸艾司西酞普兰联合阿普唑仑治疗对老年脑卒中后焦虑抑郁患者睡眠质量和神经功能的影响.方法选取西安市高陵区精神卫生中心2021年10月-2023年10月门诊加住院收治的80例老年脑卒中后焦虑抑郁患者,按照随机数字表法分为两组,每组各40例,对照组采用阿普唑仑治疗,研究组则在对照组基础上联用草酸艾司西酞普兰治疗,对比两组临床疗效.结果研究组治疗总有效率和日常生活能力(ADL)评分,以及5-羟色胺(5-HT)、多巴胺(DA)水平高于对照组,且夜间苏醒次数和神经功能缺损评估量表(NIHSS)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)、匹兹堡睡眠质量评定量表(PSQI)、阿斯森失眠量表(AIS)评分,以及S100B蛋白水平低于对照组,入睡所需时间短于对照组,睡眠维持时间长于对照组(P<0.05).结论草酸艾司西酞普兰联合阿普唑仑治疗能够促进老年脑卒中后焦虑抑郁患者神经功能和睡眠质量改善,并缓解其负性情绪和神经递质异常传导,治疗实用性较高.Objective To observe the effects of oxalate escitalopram combined with alprazolam on sleep quality and neurological function in elderly patients with post-stroke anxiety and depression.Methods 80 elderly patients with post-stroke anxiety and depression admitted to the Gaoling District Mental Health Center in Xi’an from October 2021 to October 2023 were randomly divided into two groups,each with 40 cases.The control group was treated with alprazolam,while the study group was treated with oxalate escitalopram in combination with the control group.The clinical efficacy of the two groups was compared.Results The total effective rate and daily living ability(ADL)score of the study group,as well as the levels of serotonin(5-HT)and dopamine(DA),were higher than those of the control group.In addition,the number of nighttime awakenings and neurological deficits assessment scale(NIHSS),Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD),Pittsburgh Sleep Quality Assessment Scale(PSQI),and Asen Insomnia Scale(AIS)scores were also lower in the study group,and the S100B protein level was lower than the control group,the time required to fall asleep was shorter than that of the control group,and the sleep maintenance time was longer than that of the control group(P<0.05).Conclusion The combination of escitalopram oxalate and alprazolam can promote the improvement of neurological function and sleep quality in elderly patients with post-stroke anxiety and depression,and alleviate negative emotions and abnormal neurotransmitter transmission.The treatment is highly practical.
