摘　　要：目的探讨心脏ICU洋葱伯克霍尔德菌的感染情况及耐药特点,为防治提供依据.方法选取2020-2023年赣南医学院第一附属医院心脏ICU收治的3945例患者为研究对象.采用回顾性调查方法,分析心脏ICU洋葱伯克霍尔德菌的感染率以及药敏试验结果.结果2020-2023年心脏ICU洋葱伯克霍尔德菌感染率为4.79%,微生物标本检出率为14.72%,标本来源以肺泡灌洗液为主,占73.24%;洋葱伯克霍尔德菌在检出细菌中的构成比为12.24%;药敏结果显示洋葱伯克霍尔德菌对多西环素和替加环素耐药性有上升的趋势(P<0.05),对美罗培南的耐药性有下降的趋势(P<0.05).结论心脏ICU患者洋葱伯克霍尔德菌的感染率以及标本检出率都很高,感染的部位主要是下呼吸道,临床上抗菌治疗可选用磺胺甲恶唑/甲氧苄啶、米诺环素、头孢他啶和左氧氟沙星.Objective To investigate the infection and drug resistance of Burkholderia cepacia in cardiac ICU and to provide evidence for prevention and treatment.Methods 3945 patients admitted to the cardiac ICU of the First Affiliated Hospital of Gannan Medical University from 2020 to 2023 were selected as the study objects.The infection rate and drug susceptibility test of Burkholderia cepacia in cardiac ICU were analyzed retrospectively.Results From 2020 to 2023,the infection rate of Burkholderia onion in cardiac ICU was 4.79%,and the detection rate of microbial specimens was 14.72%.The source of specimens was mainly alveolar lavage fluid(73.24%).The proportion of Burkholderia cepacia in the detected bacteria was 12.24%.The results of drug sensitivity showed that the resistance of Burkholderia cepacia to doxycycline and tigecycline increased(P<0.05),and the resistance of meropenem decreased(P<0.05).Conclusion The infection rate and detection rate of Burkholderia cepacia in cardiac ICU patients are very high,and the infection site is mainly lower respiratory tract.Clinical antimicrobial treatment can be used sulfamethoxazole/trimethoprim,minocycline,ceftazidime and levofloxacin.
