摘  要:目的:通过探究失效模式与效应分析法(failure mode and effect analysis,FMEA)在门诊智能药房药品效期管理中的应用,提高效期管理的质量。方法:应用FMEA识别智能药房药品效期管理中可能的失效模式,确定其风险优先值(risk priority number,RPN),对于RPN高的失效模式优先进行改进,通过比较改善前后的RPN值及每月近效期药品种类、数量及总金额来评价管理效果。结果:在门诊智能药房效期管理过程中共找出32个风险点,针对其中17个中高风险采取干预措施,经过6个月的整改,降至3个中风险,共下降522个RPN值,降幅为69%,近效期药品总金额由32 932.8元/月降至19 340.6元/月,P<0.05差异具有统计学意义。结论:基于FMEA法在门诊智能药房效期管理中的应用,可以有效地降低药品失效的风险,保障用药安全。OBJECTIVE To explore the application of failure mode and effect analysis(FMEA) during drug validity management of intelligent outpatient pharmacy and enhance the quality of high efficiency period management.METHODS FMEA was utilized for identifying the possible failure modes during drug validity management of intelligent pharmacy.And its risk priority number(RPN) was determined.The failure mode priority with high RPN improved and management effect was evaluated by comparing RPN value before and after improvement and type,quantity and total monthly amount of drugs during near-effective period.RESULTS A total of 32 risk points were identified during validity management of intelligent outpatient pharmacy.For 17medium-high risks,intervention measures were adopted.After 6-month rectification,value of RPN dropped to 3 medium-high risks,a total decrease of 522 RPN values,a decline of 69%.Total monthly amount of near-effective drugs dropped from 32932.8to 19 340.6 yuan.P<0.05 was statistically significant.CONCLUSION During validity management of intelligent outpatient pharmacy,application of FMEA can effectively lower the risk of drug failure and ensure drug safety.
