超声造影联合B超对良恶性肝脏病变的鉴别及诊断效能分析

摘　　要：目的:探讨超声造影联合B超对良恶性肝脏病变的鉴别及诊断效能。方法:选取2019年12月—2023年12月徐州医科大学附属医院收治的60例肝脏病变患者为研究对象,全部患者均接受B超与超声造影检查,以病理学检查结果为金标准分为肝癌组与良性病变组,比较良恶性病变的超声征象以及超声造影联合B超对肝癌的诊断效果。结果:肝癌组峰值强度高于良性病变组,造影剂到达时间、达峰时间与拟合曲度尖度低于良性病变组,差异有统计学意义(P<0.05);良性病变组28例形态规则,26例边界清晰,25例内部呈低回声;肝癌组2例形态规则,3例边界清晰,6例内部呈低回声,两组超声征象比较,差异有统计学意义(P<0.05);超声造影联合B超诊断肝癌的准确度为93.33%,灵敏度为92.86%,特异度为93.75%均高于两者单一检查,其中联合与B超单一检查比较,差异有统计学意义(P<0.05)。结论:超声造影联合B超可有效鉴别良恶性肝脏病变,相较于单一B超检查,可提升肝癌的诊断效能。Objective To investigate the differential and diagnostic efficacy of contrast-enhanced ultrasound(CEUS)combined with B-ultrasound in benign and malignant liver lesions.Methods The clinical data of 60 patients with liver lesions admitted to the Affiliated Hospital of Xuzhou Medical University from December 2019 to December 2023 were retrospectively analyzed.All patients underwent B-ultrasound and contrast-enhanced ultrasound,and divided into liver cancer group and benign lesion group based on pathological examination results as the gold standard,to compare the ultrasonic signs of benign and malignant lesions and the effect of contrast-enhanced ultrasound combined with B-ultrasound in the diagnosis of liver cancer.Results The peak intensity of liver cancer group was higher than that of benign lesion group,the arrival time of contrast agent,peak time and fitting curve sharpness were lower than those of benign lesion group,the difference was statistically significant(P<0.05).In the benign lesion group,28 cases had regular shape,26 cases had clear boundary and 25 cases had low internal echo.In the liver cancer group,2 cases had regular shape,3 cases had clear boundary,and 6 cases had low internal echo,and the ultrasonic signs of the two groups were statistically significant(P<0.05).The accuracy,sensitivity and specificity of CEUS combined with B-ultrasound in the diagnosis of liver cancer were 93.33%,92.86%and 93.75%respectively,which were higher than those of the two single examinations,and the difference between the combination and B-ultrasound was significant(P<0.05).Conclusion Contrast-enhanced ultrasound combined with B-ultrasound can effectively distinguish benign and malignant liver lesions,and can improve the diagnostic efficiency of liver cancer compared with single B-ultrasound.
