参苓白术散治疗腹泻型肠易激综合征现状分析

摘　　要：肠易激综合征(Irritable Bowel Syndrome, IBS)是一种常见的消化系统疾病,随着人们精神压力增加、饮食结构变化等,肠易激综合征在中国患病率逐渐升高,其中以腹泻型肠易激综合征(Irritable bowel syndrome with diarrhea, IBS-D)最为多见。目前现代医学治疗IBS-D虽短期疗效尚可,但仍有长期疗效欠佳、复发率高等缺陷,研究发现中医药治疗IBS-D有独特优势。参苓白术散作为治疗脾虚湿盛证IBS-D代表方之一,有益气健脾、运脾化湿的功效,现代药理学研究显示参苓白术散有调节肠道菌群、调控神经系统、修复肠屏障功能、抗炎、提高免疫等作用,可有效改善肠易激综合征胃肠道症状。该文对参苓白术散相关基础实验及临床试验进展进行综述,分析参苓白术散作用机制和临床应用,以期为提高临床疗效提供更多思路。Irritable Bowel Syndrome(IBS) is a common digestive system disease. With the increase of mental stress and the change of dietary structure, the prevalence of IBS in China is gradually increasing, and diarrhea predominant irritable bowel syndrome(IBS-D) is the most common type. At present, although the short-term efficacy of modern medicine in the treatment of IBS-D is acceptable, it has the defects of poor long-term efficacy and high recurrence rate. Studies have found that traditional Chinese medicine has unique advantages in the treatment of IBS-D. Shenling-baizhu Powder, as one of the representative prescriptions for the treatment of IBS-D with spleen deficiency and dampness, has the effect of beneficial qi and spleen strengthening, promoting spleen and removing dampness. Modern pharmacological studies have shown that Shenlingbaizhu powder can effectively improve the gastrointestinal symptoms of irritable bowel syndrome by regulating intestinal flora, regulating nervous system, repairing intestinal barrier function, anti-inflammation, and improving immunity. This article reviews the basic and clinical experiments of Shenlingbaizhu powder, and analyzes the mechanism and clinical application of Shenlingbaizhu powder, in order to provide more ideas for improving clinical efficacy.
