摘 要:目的 研究中国(超声)甲状腺影像报告和数据系统(C-TIRADS)结合不同接触面积对甲状腺乳头状癌(PTC)被膜侵犯的术前预测价值。方法 对手术病理证实的151例PTC参照C-TIRADS进行回顾性分析,并评估结节与被膜接触面积(≤25%、25%~50%、≥50%,结节与甲状腺被膜邻接长度占结节周长百分比)。结果 结节大小、接触面积及C-TIRADS分类是被膜侵犯的独立危险因素(P<0.05),建立预测模型,其曲线下面积为0.774,灵敏度为76.19%,特异度为70.15%,阳性预测值为76.19%,阴性预测值为70.15%,准确度为73.51%。结论 结节≥9.2 mm、接触面积>25%及C-TIRADS>4B类预测PTC被膜侵犯有一定的临床指导价值。Objective This study was aimed to assess the value of Chinese thyroid imaging reporting and data system(C-TIRADS) combined with different contact areas in the preoperative prediction of capsule invasion in patients with papillary thyroid carcinoma(PTC).Methods A retrospective analysis was performed on 151 cases of PTC confirmed by surgical pathology according to the C-TIRADS,and the contact area between the nodules and the capsule was evaluated(≤25%,25%~50%,≥50%,which was defined as the ratio of the contact length between the nodule and the thyroid capsule to the nodule circumference).Results Nodule size,contact area,and C-TIRADS classification were identified as independent risk factors for capsule invasion(P<0.05,for all).A predictive model was established,yielding an area under the curve of 0.774,sensitivity of 76.19%,specificity of 70.15%,positive predictive value of 76.19%,negative predictive value of 70.15%,and accuracy of 73.51%.Conclusions Nodules with a size of ≥9.2 mm,contact area >25%,and C-TIRADS>4B demonstrate significant clinical utility in predicting capsule invasion in patients with PTC.
