摘 要:目的 探讨超声引导下细针穿刺活检(US-FNAB)联合液基细胞学对腮腺肿物的诊断价值。方法 回顾性分析腮腺肿物293个,以术后组织病理为金标准,计算评估US-FNAB联合液基细胞学对腮腺肿物定性诊断的准确度、灵敏度、特异度以及对腮腺肿物诊断的准确度。结果 US-FNAB联合液基细胞学对腮腺肿物定性诊断的灵敏度为92.3%,特异度为98.7%,准确度为97.3%,阳性预测值为95.2%,阴性预测值为97.8%。US-FNAB联合液基细胞学诊断腮腺肿物的准确度为82.9%,对非肿瘤性病变、良性肿瘤、恶性肿瘤的诊断准确度分别为76.7%、85.9%、72.3%。结论 US-FNAB联合液基细胞学对腮腺肿物定性诊断具有较高的灵敏度和特异度,对良性肿瘤的诊断准确度高,是一种很好的获取术前病理的手段。Objective To evaluate the diagnostic performance of ultrasound-guided fine needle aspiration biopsy(US-FNAB) combined with liquid-based cytology in parotid lesions.Methods A retrospective analysis was performed on 293 cases of parotid lesions undergoing US-FNAB combined with liquid-based cytology.The accuracy,sensitivity and specificity of qualitative diagnosis of parotid lesions with US-FNAB combined with liquid-based cytology were evaluated.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of US-FNAB combined with liquid-based cytology were 92.3%,98.7%,97.3%,95.2%,and 97.8%,respectively.The accuracy of US-FNAB combined with liquid-based cytology in diagnosing parotid gland mass was 82.9%,and the accuracy of diagnosis of non-tumor lesions,benign tumors and malignant tumors were as follows:76.7%,85.9%,72.3%.Conclusions US-FNAB combined with liquid-based cytology has high clinical application value for the diagnosis of parotid lesions with high sensitivity and specificity.
