摘 要:目的 超声测量并比较不同孕周胎儿脐孔处脐静脉(UV)及静脉导管(DV)各项血流参数,分析各参数间的相关性。方法 分别选取孕24周、30周及37周各50例孕妇,测量计算胎儿脐孔处UV及DV内径、流速及流量,计算DV分流率,不同孕周上述测值进行比较及相关性分析。结果 各孕周胎儿间DV内径及脐孔处UV流速差异无统计学意义(P>0.05),DV流速及流量、脐孔处UV内径及流量、DV分流率差异有统计学意义(P<0.05)。组间两两比较,孕24周与30周及37周胎儿间脐孔处UV内径及流量、DV流速及分流率差异有统计学意义(P<0.05),孕30周与37周各参数差异无统计学意义(P>0.05)。相关性分析提示各孕周胎儿DV内径与流速均具有负相关性、与流量均具有正相关性,脐孔处UV内径与流速均具有负相关性、与流量均具有正相关性,DV分流率与脐孔处UV流量均具有负相关性。结论 妊娠中晚期胎儿脐孔处UV流量随孕周增大而增加,DV分流率随孕周增大而降低,各孕周胎儿各测量参数间均有相关性。Objective The blood flow parameters of umbilical vein(UV) and ductus venosus(DV) at different gestational weeks were calculated and analyzed by ultrasonic measurement,and the correlation between the parameters was analyzed.Methods Fifty pregnant women at the 24th,30th and 37th week of pregnancy were selected.The inner diameter,velocity and flow rate of UV at umbilical pore and the DV were measured.The shunt ratio of DV was calculated.The above measured values were analyzed among different gestational weeks and correlation analysis was conducted.Results There were no significant differences(P>0.05) in the diameter of DV and the velocity of UV at different gestational weeks,but there were statistically significant differences(P<0.05) in DV velocity and flow rate,UV diameter and flow rate at umbilical pore and DV shunt ratio.There were statistically significant differences(P<0.05) in the diameter and flow rate of UV at the umbilical pore,the velocity and shunt ratio of DV between the 24th and 30th and 37th gestational weeks.There was no significant difference(P>0.05) between the 30th and the 37th gestational weeks in all the above parameters.Correlation analysis indicated that the inner diameter of the DV at each gestational weeks was negatively correlated with the velocity and positively correlated with the flow rate;The inner diameter of the UV was negatively correlated with the velocity and positively correlated with the flow rate;The shunt ratio of DV was negatively correlated with the flow rate of UV.Conclusions The flow rate of UV at the umbilical pore increased with the increase of gestational weeks,and the shunt ratio of DV decreased with the increase of gestational weeks.There were correlations between the measured parameters at each gestational week.
