直肠转移癌的超声特征及经直肠超声引导活检的诊断价值

摘　　要：目的 分析直肠转移癌(RM)的经直肠超声(ERUS)特征,探讨ERUS引导活检在RM诊断中的价值。方法 回顾性分析27例RM患者的临床资料、ERUS表现及ERUS引导活检病理结果。结果 27例RM患者的原发肿瘤依次为胃癌11例(40.7%)、膀胱癌6例(22.2%)、前列腺癌4例(14.8%)、卵巢癌3例(11.1%)、肺癌2例(7.4%)、宫颈癌1例(3.7%)。RM的ERUS表现为浸润生长型23例(85.2%),肿块型4例(14.8%)。肠壁分层征和同心圆征分别存在于70.4%和75.0%的病灶,以黏膜下层及固有肌层增厚为主。所有患者均顺利完成ERUS引导活检,病理均诊断为恶性肿瘤。结论 RM的ERUS表现以浸润生长型为主,环周肠壁增厚并显示分层征和同心圆征,以黏膜下层及固有肌层增厚为主,ERUS引导活检有助于提高诊断的准确度。Objective To analyze the ultrasonic features of rectal metastasis(RM) and to investigate the value of endorectal ultrasound(ERUS) guided biopsy in the diagnosis of RM.Methods The clinical data,ERUS findings and ERUS-guided biopsy in 27 cases of RM were retrospectively analyzed.Results The primary tumors were gastric cancer in 11 cases(40.7%),bladder cancer in 6 cases(22.2%),prostate cancer in 4 cases(14.8%),ovarian cancer in 3 cases(11.1%),lung cancer in 2 cases(7.4%) and cervical cancer in 1 case(3.7%).The ERUS findings of RM showed infiltrative growth type in 23 cases(85.2%) and mass type in 4 cases(14.8%).The long-axis layered sign and the cross-sectional concentric sign were respectively showed in 70.4% and 75.0% of lesions.The thickening of the intestinal wall mainly occurred in the submucosa and muscularis propria layers.ERUS guided biopsy was successfully performed in all patients,and the pathology was diagnosed as malignant tumor.Conclusions ERUS features of RM include infiltrative growth,thickening of circumferential rectal wall with long-axis layered and cross-sectional concentric signs,mainly in the submucosa and muscularis propria layers.ERUS guided biopsy is helpful for diagnosis.
