阴道分泌物检验在念珠菌阴道炎诊断中的应用效果研究

摘　　要：研究阴道分泌物检验在念珠菌阴道炎诊断中的应用价值。以贵州省六枝特区妇幼保健院2022年9月—2023年8月收治的200例疑似念珠菌阴道炎患者为研究对象。对疑似念珠菌阴道炎患者分别实施生理盐水湿片检查、生理盐水湿片检查联合革兰氏染色后显微镜人工镜检,以临床诊断结果作为“金标准”,比较两种检查方式的疾病检出情况。结果显示,生理盐水湿片检查的阳性检出率为54.50%、灵敏度为81.60%、特异度为90.67%、阳性预测值为93.58%、阴性预测值为74.73%、诊断符合率为85.00%;生理盐水湿片检查联合革兰氏染色后显微镜人工镜检的阳性检出率为62.00%、灵敏度为96.00%、特异度为94.67%、阳性预测值为96.77%、阴性预测值为93.42%、诊断符合率为95.50%。灵敏度、特异度、诊断符合率比较,均是生理盐水湿片检查联合革兰氏染色后显微镜人工镜检疾病检出情况更加理想(P<0.05)。研究发现,在念珠菌阴道炎诊断中,采取生理盐水湿片检查联合革兰氏染色后显微镜人工镜检较单一生理盐水湿片检查方式效果更佳,值得推广应用。Study the application value of vaginal secretion testing in the diagnosis of candidal vaginitis.The study focuses on 200 patients suspected of having candidal vaginitis admitted to the Liuzhi Special Zone Maternal and Child Health Hospital from September 2022 to August 2023.For patients suspected of having candidal vaginitis,physiological saline wet slide examination and physiological saline wet slide examination combined with Gram staining and manual microscopy were performed separately.The clinical diagnosis results were used as the"gold standard"to compare the disease detection of the two examination methods.The results showed that the positive detection rate of physiological saline wetfilm examination was 54.50%,the sensitivity was 81.60%,the specificity was 90.67%,the positive predictive value was 93.58%,the negative predictive value was 74.73%,and the diagnostic accuracy rate was 85.00%;The positive detection rate of physiological saline wet slide examination combined with Gram staining and manual microscopic examination was 62.00%,sensitivity was 96.00%,specificity was 94.67%,positive predictive value was 96.77%,negative predictive value was 93.42%,and diagnostic accuracy was 95.50%.The sensitivity,specificity,and diagnostic accuracy were compared,and it was found that the combination of physiological saline wet slide examination and Gram staining with manual microscopic examination resulted in more ideal disease detection(P<0.05).Research has found that in the diagnosis of candidal vaginitis,the combination of physiological saline wet slide examination and Gram staining with manual microscopy is more effective than the single physiological saline wet slide examination method,and is worthy of promotion and application.
