摘　　要：目的观察手法松解术联合医用几丁糖治疗冻结期肩周炎的临床疗效。方法选择102例肩周炎患者,随机分为对照组和观察组。对照组采用针灸、推拿治疗,观察组采用手法松解术联合医用几丁糖治疗,2组治疗前、治疗1天、1周、1个月、3个月、6个月后,分别记录视觉模拟评分(VAS)、Constant肩关节评分(CMS)、肩关节活动度、总有效率及复诊率。结果治疗1天、1周、1个月、3个月、6个月后,2组VAS评分、CMS、肩关节活动度均有不同程度改善,且观察组明显优于对照组(P<0.05);观察组临床疗效总有效率(98%,49/50)高于对照组(78%,39/50)(P<0.05),复诊率低于对照组(P<0.05)。结论在“筋主骨从”理论指导下手法松解术联合医用几丁糖治疗冻结期肩周炎可以明显降低患者的疼痛程度、改善肩关节功能障碍、提高肩关节活动度、降低复诊率,在改善患者功能障碍方面优势明显。Objective To observe the clinical efficacy of manipulation combined with medical chitosan in the treatment of frozen shoulder periarthritis.Methods 102 patients with periarthritis of shoulder were selected and divided into the control group and the observation group.The control group was treated with acupuncture and moxibustion and massage,and the observation group was treated with manipulation combined with medical chitosan.Before treatment,and after 1 day,1 week,1 month,3 months,and 6 months of treatment,Visual Analogue Scale(VAS)score,Constant-Murley Score(CMS),shoulder joint range of motion,total eff ective rate,and further consultation rate were recorded in two groups.Results After 1 day,1 week,1 month,3 months,and 6 months of treatment,both groups showed varying degrees of improvement in VAS scores,CMS scores,and shoulder joint range of motion,and the observation group was significantly better than the control group(P<0.05);The total effective rate of the observation group(98%,49/50)was higher than that of the control group(78%,39/50),and the further consultation rate of the observation group was lower than that of the control group(P<0.05).Conclusion Under the guidance of the theory of tendons domination and bones subordination,the combination of manipulation and medical chitosan can significantly reduce the pain of patients with frozen shoulder periarthritis,improve shoulder joint dysfunction,increase shoulder joint mobility,and reduce further consultation rate.This therapy has signifi cant advantages in improving the dysfunction of patients with periarthritis of shoulder.
