梧州市实施艾滋病病例综合管理模式的效果分析

摘　　要：梧州市通过由医疗机构、疾病预防控制中心、社区卫生服务机构(乡镇卫生院)、定点抗病毒治疗点共同制定联动对接转介机制,建立起艾滋病病例综合管理模式。该工作模式各工作环节由专人负责,充分有效利用各部门医疗资源及服务特长,最大程度地发现艾滋病病例,并对艾滋病病例进行综合管理与救治。自从2020年梧州市建立艾滋病病例综合管理模式后,艾滋病筛查人次数每年同比增加;初筛阳性病例送检确证率由2019年的42.42%增加至2020年的72.85%(χ^(2)=134.525,P<0.001),之后3年每年持续增加(χ_(趋势)^(2)=114.558,P<0.001),2022年达96.37%;艾滋病病例随访CD4检测率及配偶检测率每年均达90.00%的要求;艾滋病病例抗病毒治疗率由2019年的78.69%增加至2020年的91.33%(χ^(2)=223.374,P<0.001),2020—2022年继续保持在90.00%以上。艾滋病病例综合管理模式对梧州市艾滋病病例的及早发现和管理发挥了重要作用,可推广应用。The comprehensive management model of Acquired Immune Deficiency Syndrome(AIDS)cases was developed with the collaborated referral approach established by related medical institutions,center for disease control and prevention,community health service institutions(hospitals in townships),dedicated anti-virus&cure spots.Each processing steps of this model was assigned to responsible people,it was efficiently utilizing medication resources and expertise in various departments.More and more AIDS cases could be identified to the most extent,and could be comprehensively managed and cured.From 2020,after the establishment of the comprehensive management model of AIDS cases,the number of Human Immunodeficiency Virus(HIV)test has increased year on year.Since 2019 to 2020,the positive confirmation rates raised from 42.42% to 72.85%(χ^(2)=134.525,P<0.001),showing an increasing trend from 2020 to 2022(χ_(Trendliness)^(2)=114.558,P<0.001),and reached 96.37% in 2022.The follow up CD4 test rates and the test rates of the spouses of AIDS cases all meet 90% every year.Since 2019 to 2020,the anti-viral treatment rate of AIDS cases was increased from 78.69% to 91.33%(χ^(2)=223.374,P<0.001),and kept above 90% from 2020 to 2022.Comprehensive management model of AIDS cases was effective and feasible locally,which could be implemented in further.
