摘 要:目的应用定量CT(QCT)技术测量腰椎骨密度(BMD),研究中老年男性体检人群血红蛋白、血尿酸与腰椎BMD的相关性。方法选取2022年1月至2023年2月在河南省人民医院健康管理学科行胸部CT联合QCT检查的3611例中老年男性体检者为研究对象,搜集其年龄、体质量指数(BMI)、血红蛋白、血尿酸和血脂指标,同时使用QCT测量受试者腰椎BMD。根据腰椎BMD值分为骨量正常组(>120 mg/cm^(3))、骨量减少组(80~120 mg/cm^(3))和骨质疏松组(<80 mg/cm^(3))。采用单因素方差分析、Pearson相关性分析和多因素二元Logistic回归模型分析血红蛋白、血尿酸与腰椎BMD的相关性。结果不同腰椎BMD组间年龄、BMI、血红蛋白、总胆固醇、甘油三酯、低密度脂蛋白、血尿酸比较差异有统计学意义(P<0.05)。Pearson相关性分析结果显示BMI、血红蛋白、总胆固醇、甘油三酯、低密度脂蛋白和血尿酸与腰椎BMD呈正相关(r=0.049、0.162、0.090、0.066、0.077、0.064,P<0.05),年龄与腰椎BMD呈负相关(r=-0.410,P<0.001)。多因素二分类Logistic回归结果显示,在调整其他变量的影响下,血红蛋白与血尿酸轻度升高是中老年男性骨质疏松症的保护因素(OR=0.978,95%CI:0.969~0.987;OR=0.998,95%CI:0.997~1.000;P<0.05)。结论在中老年男性体检人群中,血红蛋白、血尿酸与腰椎BMD存在相关性,血红蛋白与血尿酸是中老年男性体检人群骨质疏松症的影响因素。Objective To explore the correlation between hemoglobin,serum uric acid and lumbar spine bone mineral density(BMD)in middle-aged and elderly male physical examination population based on Quantitative CT(QCT).Methods A total of 3611 middle-aged and elderly male physical examination population who underwent chest CT combined with QCT examination in the Health Management Discipline of Henan Provincial People's Hospital from January 2022 to February 2023 were selected as the study subjects,and their age,body mass index(BMI),hemoglobin,serum uric acid and blood lipid indexes were collected,and lumbar spine BMD was measured by QCT.According to the result of BMD,the subjects were divided into normal bone mass group(>120 mg/cm^(3)),reduced bone mass group(80~120 mg/cm^(3))and osteoporosis group(<80 mg/cm^(3)).Univariate ANOVA,Pearson correlation and multivariate binary logistic regression model were used to analyze the correlation between hemoglobin,serum uric acid and lumbar spine BMD.Results There were significant differences in age,BMI,hemoglobin,total cholesterol,triglycerides,low-density lipoprotein and serum uric acid between different lumbar spine BMD groups(P<0.05).The results of Pearson correlation analysis showed that BMI,hemoglobin,total cholesterol,triglycerides,low-density lipoprotein and serum uric acid were positively correlated with lumbar spine BMD(r=0.049,0.162,0.090,0.066,0.077,0.064,P<0.05),but age was negatively correlated with lumbar spine BMD(r=-0.410,P<0.001).The results of multivariate binary Logistic regression showed that hemoglobin and slightly elevated serum uric acid were the protective factors for osteoporosis in middle-aged and elderly men after adjusting for other variables(OR=0.978,OR95%CI:0.969~0.987,OR=0.998,OR95%CI:0.997~1.000).Conclusion In middle-aged and elderly male physical examination population,hemoglobin and serum uric acid are correlated with lumbar spine BMD,and hemoglobin and serum uric acid are the influencing factors of osteoporosis in middle-aged and elderly male physica
