D-D、FDP连续监测结合骨折时间在老年股骨近端骨折患者中的应用

摘  要:目的探讨血浆D-二聚体(D-D)和纤维蛋白降解产物(FDP)的连续检测结合骨折时间对老年股骨近端骨折患者下肢深静脉血栓(DVT)的预测价值。方法回顾性分析241例2023年1月至6月间老年股骨近端骨折患者作为观察组,其中男117例,女124例,纳入100例同期老年健康体检者为对照组。通过连续动态监测手术前后血浆D-D和FDP的水平,结合患者骨折时间,对下肢DVT的发生情况进行预测价值分析。结果观察组术前血浆D-D、FDP水平高于对照组(P<0.05);发生DVT患者术后连续监测显示,血浆D-D、FDP水平均高于未发生DVT患者(P<0.05)。骨折时间越长,发生下肢DVT的风险越高(P<0.05)。结论D-D和FDP的连续检测可密切反映体内凝血及纤溶的状况,骨折时间是下肢DVT的危险因素,连续监测D-D、FDP,结合骨折时间有助于预测评估老年股骨近端骨折患者DVT的发生风险。Objective To explore and analyze the predictive value of joint detection of fracture duration,plasma D-dimer(D-D),and fibrinogen degradation products(FDP)for lower limb deep vein thrombosis(DVT)in patients with proximal femoral fractures.Methods Retrospective analysis was performed for 241 aged patients with proximal femoral fractures from January 2023 to June 2023,including 117 males and 124 females,with 80 healthy people who underwent health examinations during the same period served as the control group.Continuous dynamic monitoring of plasma D-D and FDP levels before and after surgery together with the duration of fractures,were statistically analyzed the predictive value of occurrence of lower limb DVT.Results The results showed that the postoperative plasma D-D and FDP levels in the observation group were higher than those in the control group(P<0.05).Within the observation group,the postoperative plasma D-D and FDP levels in patients with DVT were higher than those without DVT(P<0.05).The longer the duration of fracture,the higher the risk of lower limb DVT for postoperative patients(P<0.05).Conclusion Continuous dynamic detections of FDP and D-D can closely monitor the coagulation and fibrinolysis status in the body with the duration of fracture as a risk factor for lower limb DVT,which is helpful for the early diagnosis of DVT in postoperative patients with fractures.
