摘 要:目的探讨微创内固定术与切开复位内固定术对四肢骨折患者围术期指标及术后关节功能恢复的影响。方法选择2021年8月至2022年12月于平顶山市第一人民医院骨科住院治疗的64例四肢骨折患者为研究对象,根据随机数字表法分为常规组与研究组,每组32例。常规组行切开复位内固定术治疗,研究组行经皮微创内固定术治疗。比较两组手术相关指标(手术时间、术中出血量、切口长度、住院时间、骨折愈合时间)、血清应激指标[皮质醇(Cor)、白细胞介素-6(IL-6)、C-反应蛋白(CRP)]水平、关节活动度、关节功能恢复优良率及术后并发症。结果研究组手术时间、术中出血量、切口长度、住院时间、骨折愈合时间均明显小于常规组,差异有统计学意义(P<0.05)。两组术后Cor、IL-6、CRP水平均升高,但研究组低于常规组,差异有统计学意义(P<0.05)。术后1、2、3个月,研究组关节活动度均优于对照组,差异有统计学意义(P<0.05)。术后3个月,研究组关节功能恢复优良率明显高于常规组,差异有统计学意义(P<0.05)。研究组术后并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论微创内固定术治疗四肢骨折疗效显著,可减轻手术创伤及手术应激,促进患者骨折愈合和关节功能恢复,且安全性高。Objective To investigate the effects of minimally invasive internal fixation and open reduction and internal fixation on perioperative indicators and postoperative joint function recovery in patients with limb fractures.Methods Sixty⁃four patients with limb fracture hospitalized in the FirstPeople’s Hospital of Pingdingshan from August 2021 to December 2022 were selected and divided into the conventional group and the study group,according to the random number table method,with 32 cases in each group.The conventional group underwent open reduction and internal fixation,and the study group underwent percutaneous minimally invasive internal fixation.The operation related indexes(operation time,intraoperative blood loss,incision length,hospital stay,fracture healing time),serum stress indexes[cortisol(Cor),interleukin⁃6(IL⁃6),C⁃reactive protein(CRP)],joint range of motion,excellent and good rate of joint function recovery and postoperative complications were compared between the two groups.Results The operation time,intraoperative blood loss,incision length,length of hospital stay,fracture healing time were significantly less in the study group than those in the conventional group(P<0.05).The postoperative levels of Cor,IL⁃6 and CRP were higher than those before operation in the two groups,but the above indexes were lower in the study group than those in the conventional group(P<0.05).At 1,2,and 3 months after operation,the joint activity of the study group was better than that of the control group(P<0.05).The excellent and good rate of joint function recovery in the study group was significantly higher than that in the conventional group at 3 months after operation(P<0.05).The incidence of postoperative complications were significantly lower in the study group than those in the control group(P<0.05).Conclusions Minimally invasive internal fixation is effective on limb fractures,which can reduce surgical trauma and stress,promote fracture healing and joint function recovery,and has high safety.
