甲亢治疗后并发甲减的治疗分析

摘　　要：目的分析甲状腺功能亢进(简称甲亢)治疗后并发症甲状腺功能减退(简称甲减)的发生情况,并总结预防措施,用以指导后续的临床工作。方法选取2021年1月—2022年1月滕州市中心人民医院收治的甲亢患者102例,根据治疗方式将其分为研究组和对照组,每组各51例,研究组采用甲巯咪唑药物治疗,对照组采用131I放射治疗。比较两组甲减的发生情况。结果治疗后,研究组患者出现甲减1例,出现亚临床甲减1例,合计2例。对照组患者出现甲减8例,出现亚临床甲减2例,合计10例。研究组甲减发病率为3.92%,低于对照组的19.61%,差异有统计学意义(χ^(2)=6.044,P<0.05)。12例甲减患者FT3低于正常值7例,FT4低于正常值9例,TSH高于正常值10例。结论对于使用131I放射治疗的甲亢患者,在临床中需要及时监测患者的各项指征,并做好后续的随访工作。相较于131I放射治疗,应尽早服用甲巯咪唑治疗,可以有效降低甲减的发生率,具有较高的临床应用价值。Objective To analyze the incidence of complications of hypothyroidism after hyperthyroidism treatment and summarize preventive measures to guide subsequent clinical work.Methods A total of 102 patients with hyperthyroidism admitted to Tengzhou Central People's Hospital from January 2021 to January 2022 were selected.They were divided into study group and control group based on the treatment method,with 51 patients in each group.The study group received treatment with methimazole,while the control group received 131I radiation therapy.The occurrence of hypothyroidism between two groups were compared.Results After treatment,there was 1 case of hypothyroidism and 1 case of subclinical hypothyroidism in the study group,totaling 2 cases.There were 8 cases of hypothyroidism and 2 cases of subclinical hypothyroidism in the control group,totaling 10 cases.The incidence rate of hypothyroidism in the study group was 3.92%,which was lower than 19.61%in the control group,and the difference was statistically significant(χ^(2)=6.044,P<0.05).Among the 12 patients with hypothyroidism,7 cases had FT3 below normal,9 cases had FT4 below normal,and 10 cases had TSH above normal.Conclusion For patients with hyperthyroidism treated with 131I radiation therapy,timely monitoring of the patient's various indications and follow-up work were necessary in clinical practice.Compared to 131I radiation therapy,early use of methimazole therapy could effectively reduce the incidence of hypothyroidism,which had higher clinical application value.
