狼疮性脂膜炎合并盘状红斑狼疮伴钙沉着1例

摘　　要：患者女,54岁,躯干、上肢皮肤出现结节、萎缩30年,头皮、躯干红斑2年。皮肤科情况:躯干及双上肢可见多处皮肤萎缩,萎缩处皮下可扪及结节;头皮、躯干部可见紫红色斑块。躯干部萎缩皮损病理表现为表皮部分基底细胞液化变性,真皮胶原纤维间可见黏蛋白沉积,附属器周围、脂肪小叶及间隔内可见以淋巴细胞为主的炎症细胞浸润;患者头皮皮损组织病理表现为表皮基底细胞液化变性,附属器周围淋巴细胞为主的炎细胞浸润;胸部CT检查结果示:胸背部及左侧腋下皮下多发高密度影,部分高密度影间可见点状钙化灶。结合临床表现及辅助检查结果诊断:狼疮性脂膜炎合并盘状红斑狼疮伴钙沉着,系统给予羟氯喹片0.2 g,2次/d,红斑局部予以卤米松乳膏外用,目前患者随访中。A 54-year-old female presented with a 30-year history of skin nodules and atrophies on the trunk and upper limbs,and a 2-year history of erythema on the scalp and trunk.Dermatological examination revealed various sizes of skin atrophy in the trunk and both upper limbs,and nodules could be detected under the skin at the atrophic places.Purpish red plaques could be seen on the scalp and trunk.The histopathology of atrophic skin lesions on the trunk showed liquefaction and degeneration of partial basal cells in the epidermis,mucin deposition can be seen between the dermal collagen fibers,and inflammatory cells dominated by lymphocytes around the appendages and in the fat lobules and septa were infiltrated.The histopathology of the patient′s scalp lesions were liquefaction and degeneration of basal cells in the epidermis and inflammatory cells dominated by lymphocytes around the appendages.The chest CT scan demonstrated multiple high-density shadows on the chest,back,and left armpit,with some spotty calcifications between the shadows.Combined with the clinical manifestations and auxiliary examination results,the case was diagnosed as lupus panniculitis with discoid lupus erythematosus and calcinosis cutis.The patient was treated with hydroxychloroquine 0.2 g twice a month and topical halomethasone cream.The patient was currently being followed-up.
