尿酸/白蛋白比值联合三酰甘油葡萄糖乘积指数对早期慢性肾脏病的诊断价值

摘　　要：目的探讨尿酸/白蛋白比值(UAR)联合三酰甘油葡萄糖乘积(TyG)指数对早期慢性肾脏病(CKD)的诊断价值。方法比较早期CKD患者108例(A组)和同期健康体检者78例(C组)的血液指标。多因素logistic回归分析早期CKD发生的危险因素。绘制ROC曲线评估UAR和TyG指数单独或联合检测对早期CKD的诊断效能。结果与C组比较,A组WBC、中性粒细胞计数(Neu)、平均红细胞血红蛋白浓度、红细胞分布宽度变异系数、TC、TG、LDL-C、BUN、SCr、尿酸(UA)、UAR及TyG指数均升高,而Hb、平均红细胞体积、总蛋白、白蛋白(Alb)及FBG均降低(P<0.01或P<0.05)。Neu、TG、UA、UAR及TyG指数升高和Hb、Alb降低是早期CKD发生的独立危险因素(P<0.01或P<0.05)。UAR联合TyG指数诊断早期CKD的AUC为0.933,灵敏度和特异度分别为79.07%和97.44%。结论UAR和TyG指数升高是早期CKD发生的独立危险因素,二者联合检测对早期CKD具有一定的诊断价值。Objective To explore the diagnostic value of uric acid-albumin ratio(UAR)combined with triglyceride-glucose(TyG)index in the early stage of chronic kidney disease(CKD).Methods The blood indicators of 108 patients in the early stage of CKD(group A)and 78 healthy controls(group C)were compared.Multivariate logistic regression analysis of the risk factors for CKD in the early stage was performed.ROC curve was drawn to evaluate the diagnostic efficiency of UAR and TyG index alone or in combination for CKD in early stage.Results WBC,neutrophil count(Neu),mean corpuscular hemoglobin concentration,coefficient variation of red blood cell distribution width,TC,TG,LDL-C,BUN,SCr,uric acid(UA),UAR and TyG index were higher,while Hb,mean corpuscular volume,total protein,albumin(Alb)and FBG were lower in group A than those in group C(P<0.01 or P<0.05).Higher Neu,TG,UA,UAR and TyG index and lower Hb and Alb were the independent risk factors for CKD in the early stage(P<0.01 or P<0.05).The AUC of UAR combined with TyG index for diagnosis of CKD in the early stage was O.933,with sensitivity of 79.07%and specificity of 97.44%,respectively.Conclusion Higher UAR and TyG index are the independent risk factors for CKD in the early stage.Combined use of UAR and TyG index has a certain diagnostic value for that.
