摘 要:目的研究便携式睡眠监测(PM)方法在儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)诊断中的临床应用效果。方法选择2021年07月~2023年001月我院收治的由于腺样体肥大和(或)扁桃体肥大引起疑似OSAHS的94例患儿,所有患儿均采用多导睡眠监测仪(PSG)监测以及PM监测。统计PSG监测以及PM监测对疑似患有OSAHS的94例患儿的监测结果;监测分析指标主要包括:AHI、睡眠效率、平均血氧饱和度(MSaO2)和最低动脉血氧饱和度(LSaO2);且统计PM监测方法的灵敏度、准确度和特异度。结果PSG监测诊断为患有单纯性鼾症者有12例,诊断患有OSAHS者有82例(轻度OSAHS26例、中度OSAHS41例、重度OSAHS15例);PM监测诊断为患有单纯性鼾症者有15例,诊断患有OSAHS者有79例(轻度OSAHS24例、中度OSAHS40例、重度OSAHS15例):PSG监测以及PM监测的AHI、睡眠效率、MSaO2和LSaO2相比无明显差异(P>0.05);PSG是诊断OSAHS的金标准,以本研究中PSG监测的结果作为标准,PM监测方法的灵敏度为93.14%、准确度为96.34%、特异度为95.72%。结论PSG监测以及PM监测的各项指标具有较好的相关性,PM监测的操作方法比较简便,有助于检查的便利性和及时性,而且能明显降低患者的医疗支出,容易被患者及其家属接受,在儿童OSAHS的诊断中具有较高的应用价值。Objective To investigate the clinical application effect of portable sleep monitoring(PM)method in the diagnosis of obstructive sleep apnea hypopnea syndrome(OSAHS)in children.Methods 94 children with suspected OSAHS due to adenoid hypertrophy and/or tonsil hypertrophy admitted to our hospital from January 2018 to December 2020 were selected.All children were monitored by polysomnography(PSG)and PM monitor.Statistical PSG monitoring and PM monitoring of 94 children suspected of having OSAHS;monitoring and analysis indicators mainly include:AHI,sleep efficiency,mean blood oxygen saturation(MSaO2)and minimum arterial blood oxygen saturation(LSaO2);and statistical PM monitoring method sensitivity,accuracy and specificity.Results PSG monitoring diagnosed with simple snoring in 12 cases,diagnosed with OSAHS in 82 cases(mild OSAHS in 26 cases,moderate OSAHS in 41 cases,severe OSAHS in 15 cases);PM monitoring in the diagnosis of simple snoring in 15 cases,79 cases were diagnosed with OSAHS(24 cases of mild OSAHS,40 cases of moderate OSAHS,15 cases of severe OSAHS).There was no significant difference between PSG monitoring and PM monitoring AHI,sleep efficiency,MSaO2 and LSaO2(P>0.05).PSG was the gold standard for diagnosing OSAHS.Based on the results of PSG monitoring in this study,the sensitivity of PM monitoring method is 93.14%,accuracy is 96.34%,and specificity is 95.72%.Conclusion The various indicators of PSG monitoring and PM monitoring have a good correlation.The operation method of PM monitoring is relatively simple,which helps the convenience and timeliness of examination,and can significantly reduce the medical expenditure of patients,which is easy to be affected by patients and their families.Accepted,it has high application value in the diagnosis of children's OSAHS.
