心率变异性与老年慢性心衰严重程度的相关性分析

摘　　要：目的基于对心率变异性相关指标的分析,探讨其与老年慢性心力衰竭严重程度的相关性。方法选取我院90例老年慢性心力衰竭患者作病例组,按NYHA心功能分级,分为心功能Ⅱ级组、心功能Ⅲ级组、心功能Ⅳ级组,选取30例健康老年人群作对照组,所有研究对象均行动态心电图检查,病例组另外进行了心脏超声及NT-pro BNP检查。分析各组之间的HRV指标差异以及病例组中HRV指标和射血分数、NT-pro BNP的相关性。结果心率变异性指标在对照组和不同心功能组间均不全相同,随着NYHA心功能分级的增加,HRV下降程度越明显。心率变异性指标与射血分数呈正相关。心率变异性指标与NT-pro BNP呈负相关,其中SDNN与NT-pro BNP相关系数为-0.697,有较强负相关。结论心率变异性可用于提示老年慢性心力衰竭严重程度。Objective To explore the correlation between heart rate variability(HRV)and the severity of elderly patients with chronic heart failure based on the analysis of heart rate variability(HRV).Methods Ninty elderly patients with chronic heart failure in our hospital were selected as case group.According to NYHA cardiac function classification,90 elderly patients were divided into cardiac function classⅡgroup,cardiac function classⅢgroup and cardiac function classⅣgroup.Thirty healthy elderly people were selected as control group.All the subjects were examined by dynamic electrocardiogram,and the case group was examined by cardiac color ultrasound and NT-proBNP.The difference of HRV index among each group and the correlation between HRV index,ejection fraction and NT-proBNP in the case group were analyzed.Results The indexes of HRV were different between the control group and different cardiac function groups.With the increase of NYHA cardiac function grade,the decrease of HRV was more obvious.There was a positive correlation between HRV index and ejection fraction.There is a negative correlation between HRV and NT-proBNP,and the correlation coefficient between SDNN and NT-proBNP is-0.697,which has a strong negative correlation.Conclusions HRV can be used to indicate the severity of elderly patients with chronic heart failure.
