肖京主任诊治颈源性肩背痛的临床经验

摘　　要：颈源性肩背痛临床较常见但误诊率较高。肖京主任认为颈椎、胸椎周围的筋结点是诊断与治疗颈源性肩背痛的关键。基于《黄帝内经》“分肉之间”理论与经筋医学,筋结的形成是风寒湿邪侵袭分肉之间,导致分肉之间的正常空间消失,肌肉发力出现互相干涉,局部力线异常,从而导致疼痛及活动障碍。通过对筋结点施以刃针合谷刺治疗,恢复分肉之间的正常空间,同时配合按摩松筋手法与颈椎旋提法或扩胸扳法,调节脊柱整体力线,从而改善患者疼痛症状与颈背部活动度。本文总结了肖京主任诊治颈源性肩背痛的临床经验及治疗思路,为该病的临床实践提供参考。Cervicogenic shoulder and back pain is clinically common but has a high misdiagnosis rate.Director Xiao Jing believes that the tendon nodes around the cervical and thoracic vertebrae are the key to the diagnosis and treatment of cervicogenic shoulder and back pain.Based on the theory of“muscular clearance”in Huangdi Neijing and meridian medicine,the formation of tendon knots is caused by wind,cold,and dampness attacking between the muscular clearance,resulting in the disappearance of normal space between the muscular clearance,interfering with each other’s muscle force,and abnormal local force lines,which leads to pain and impaired mobility.By applying bladed needle hégǔstabbing treatment to the tendon nodes,the normal space between the muscular clearance is restored,and at the same time,combined with massage and relaxation techniques and cervical spine rotation and lifting method or expanding the chest wrenching method,the overall force line of the spine is adjusted,so as to improve the patient’s pain symptoms and the mobility of the back of the neck.This paper summarizes the clinical experience and therapeutic ideas of Director Xiao Jing in the diagnosis and treatment of cervicogenic shoulder and back pain,and provides a reference for the clinical practice of this disease.
